Mental Health and Substance Abuse Case Manager

**Life Landscaping, Inc. is searching for a motivated, organized, fast-paced, and articulate individual to work as a Case Manager for one of our mental health and substance abuse IOP Treatment Centers in Los Angeles.**
This position requires you to be assertive, and organized, work without requiring constant guidance, and be able to manage a comfortable caseload. You must be knowledgeable and efficient in all Microsoft Office programs, and Kipu Electronic Medical Records. Please have experience with the intake and admissions process and be able to help wherever needed. Life Landscaping, Inc. is a rapidly growing business in mental health and substance abuse treatment and we need a rock star Case Manager to hit the ground running
**Principal Responsibilities**:
Work collaboratively with treatment team members
Responsible for providing individual case management for patients in an Intensive Outpatient setting.
Create a Client Intake Care Plan for the first 2 weeks of treatment, and subsequently thereafter.
Responsible for making appointments to address client needs, that includes but is not limited to court dates, hearings, medical appointments etc.
Coordinate transportation, with transportation driver, to outside appointments
Establish Release of Authorizations for all responsible parties in patient's case, including but not limited to social workers, probation officers, medical doctors, psychiatrist, case workers, health care
Responsible for working collaboratively with Treatment Team in the best interest of the clients served
Establish relationships with community-based providers for referrals
Conduct daily case management sessions with new intakes and your case load
Facilitate groups
Present status of Case Management Care Plan in weekly Case Conference Meeting
Collect necessary data for reporting purposes; complete reports as assigned and submit to Clinical Director.
Discusses all patient information only with appropriate program personnel when related to the care being provided.
Maintains confidentiality for all documents that contain patient identifier information and is familiar with HIPPA compliance.
Observes and respects confidentiality of information in regards to fellow employees.
Maintains standards of cleanliness, organization for work station, community room and other areas of use
Completes addiction and cultural competency trainings effectively and efficiently, and other required trainings each fiscal year.
Demonstrates courtesy, mutual respect and compassion in dealing with others.
Initiates and presents positive solutions to problems.
Projects a positive professional image and adheres to organizational dress code.
Performs other duties as assigned.
**Requirements**:
1. Certified Substance Use Disorder (SUD) Counselor: Must be certified by one of the agencies accredited by the NCCA and recognized by DHCS, which include CADTP and CCAPP; and who must adhere to all requirements of the California Code of Regulations, Title 9, Chapter 8 and Title 9.
2. Two Year experience as a case manager
3. Able to navigate Electronic Heath Recording Systems (KIPU)
4. Experience with administering the ASAM, ASI and other substance abuse assessment tools
5. Must have valid California driver's license, proof of insurance and excellent driving record
6. Excellent written, oral, organizational and documentation skills
7. Perform multiple tasks while demonstrating the ability to work within a team structure
8. Must maintain a current First Aid/CPR Certification, physical exam, and Tuberculosis test results.
**Preferred**:
1. Experience in residential treatment setting working with minority and low-income pregnant women and children.
2. Experience working with an inter disciplinary team (LCSW, MDs, Counselors, Case Managers)
3. Experience working with substance abuse, mental health and co-occurring disorders
