Case Manager Needed – Part-Time

As a SUD Case Manager/Counselor, you will play a vital role in providing individualized care and support to clients with substance abuse and dual diagnosis disorders. Your expertise and dedication will help clients on their path to recovery, making a lasting impact on their lives.


Job Duties:

  • Creating a Welcoming Environment: You will be the friendly face that orients new clients to the facility's rules and expectations, ensuring they feel comfortable and supported from day one.
  • Personalized Care: Providing weekly individual Substance Abuse and Case Management sessions, you will build strong connections with your assigned clients, understanding their unique needs and collaborating on treatment plans to achieve their goals. Provide family collaboration sessions when needed.
  • Guiding Recovery Groups: Leading groups on topics relevant to addiction recovery, you'll create a safe and empowering space for clients to share their experiences. Family group may be included.
  • Collaborative Approach: Working closely with the interdisciplinary team, you will ensure the seamless coordination of relevant services, fostering integrated care that makes a genuine difference.
  • Empowering Treatment Plans: Your creativity and expertise will shine as you develop and implement individualized treatment plans, setting meaningful goals and measurable objectives for each client's journey.
  • Being There in Times of Crisis: Your compassion will be a guiding light as you offer crisis intervention to clients whenever needed, providing crucial support during challenging moments.
  • Thorough Assessment and Documentation: Conducting screenings, intake assessments, and maintaining timely notes in the client's EMR are essential aspects of your role, ensuring the highest level of care.
  • Structured Support: By creating weekly schedules for each assigned client, you will help them stay on track and engaged in their recovery journey.
  • Team Collaboration: Attending weekly clinical staff meetings and group supervision with the clinical director, you will foster a supportive and collaborative work environment.
  • Effective Communication: Regularly liaising with the clinical program director and client therapist, you'll ensure smooth communication and address any concerns related to client care.
  • Professional Development: You'll attend monthly, quarterly, or scheduled staff trainings to enhance your skills and stay up-to-date with the latest industry knowledge.
  • Administrative Excellence: Administrative work, as needed, will be carried out with diligence, maintaining accuracy and efficiency.
  • Discharge and Aftercare Planning: Collaborating with the client's assigned therapist, you'll develop effective discharge and aftercare plans, ensuring continued support post-treatment.
  • Other duties as assigned


Knowledge, Skills, and Abilities Required:

  • Empowering Expertise: We are looking for someone with a strong working knowledge of substance abuse treatment and co-occurring disorders, capable of guiding clients through their recovery journey with compassion and professionalism.

  • Clear Communication: Strong verbal and written communication skills will help you connect with clients and colleagues in a clear, concise, and professional manner.

Education and Experience:

  • Seasoned Experience: A minimum of 1 year of direct client care experience in substance abuse or dual diagnosis treatment will showcase your expertise in this field.
  • Certifications: MUST HOLD current and verifiable certification as a CATC I or higher, CADC I or higher, or SUDDC I or higher is a must.
  • Safety Matters: A current CPR/First Aid Certificate and a current T.B. Test will ensure we provide the highest level of care to our clients.
  • Safe Travels: A clean driving record is essential as you may need to travel for certain aspects of the job.
  • Background Check: As we uphold the highest standards of care and professionalism, we will conduct a background check.

Expected hours: 24 hours a week


Schedule:

  • Monday to Friday ( 3 days a week – Days flexible)

Education:

  • High school or equivalent (Preferred)

Experience:

  • Substance abuse counseling and case management: 1 year (Required)

License/Certifications accepted:

  • CATC
  • CADAC
  • SUDCC
  • RADT (Must be enrolled in a SUD Counselor Certification Program)
