**Responsibilities**:

- Coordination of care with entire care team
- Completing home care nursing visits
- Educating patients and family members
- Preparing timely, complete and accurate clinical notes
- Communicating with healthcare providers and other care team members

**Qualifications**:

- ** LVN / LPN in the State of Texas**:

- **Home Health experience a must**:

- **Knowledge of Home Health regulations and general healthcare compliance**:

- **(Preferred but not required) Experience with (HCHB) Home Care Home Base EMR**:

- Truly cares for people and will go above and beyond to provide outstanding patient care
