**We are looking for a Case Manager II for our Differential Response BHSD Program**

Under the direction of the Clinical Program Manager, Program Supervisor, or Clinical Program Supervisor, and following established procedures, provides community-based and in-home services to children and families who are receiving DR services. As part of the treatment team, job performance contributes to the achievement of the goals found on the case plan and/or as developed by the CFT or treatment team. Performance of the typical responsibilities and duties requires the ability to perform the job duties, explain to/instruct others, involving job-related matters generally of limited to moderate complexity.

TYPICAL RESPONSIBILITES AND DUTIES:

- Demonstrates a high level of competence working with children and families to assist them in attaining permanency, safety, well-being, and their self-determined quality of life.
- Ability to engage clients in manner that is culturally competent and dynamic and abides by time-sensitive deadlines.
- Provides quality, culturally proficient, trauma informed and relevant ancillary services as part of a DR team, treatment team and/or Child and Family Team that is community and in-home based.
- Possesses knowledge of community-based resources and ability to link and refer families to resources in the community based on their assessed needs.
- Documents necessary information as required within the DR Program in a timely manner and consistently meets or exceeds agency productivity standards, including Intake Forms and Case Plans.
- Supports in the collection and sharing of information and data required for treatment and outcomes via the use of tools such as NCFAS-G, CANS, WFI-EZ, PSC-35 and CSQ-8.
- Maintains professional standards regarding client rights.
- Demonstrates an understanding of confidentiality policies, mandated child abuse reporting and duty to warn procedures. Utilizes clinical supervision in any case where these issues are in question.
- Demonstrates the ability to think creatively and intervenes/responds in crisis situations.
- Utilizes supervision, consultation, and training in providing on-going high-quality services to children and their families effectively and efficiently.
- Demonstrates the ability to work independently. Actively uses supervision and consultation from his/her supervisor and other team members.
- Every employee has the opportunity and responsibility to participate in one or more activities each year that support the performance quality improvement (PQI) of RCS programs and services. Activities might involve membership on a PQI committee or work group, service as a peer reviewer, data collection for PQI and evaluation purposes, completion of PQI questionnaires and surveys or participation in focus groups, or other activities as identified by PQI committees or work groups.

**MINIMUM QUALIFICATIONS**:
EDUCATION, TRAINING AND EXPERIENCE:

- Associate or bachelor’s degree in social services, human services or related (or 6 years of equivalent years of experience) preferred.
- Minimum of two (2) years of experience working in children’s mental health setting emphasizing a family systems perspective desirable.
- Must meet the Title 9, Title 19, and Title 22 Medi-Cal Regulations
- Basic skills in crisis intervention, suicide assessment, conflict resolution, and interpersonal communication.
- Bilingual/ Bicultural (Spanish/English) and/or other threshold languages preferred.
