**PIBLY Residential Programs, Inc. is a Bronx-based, not-for-profit, voluntary organization certified by the New York State Office of Mental Health. It operates residential programs which provide rehabilitative and support services to adults suffering from mental illness. We Currently have openings for multiple Case Managers at our Adult Home. Adult Home is a permanent scatter-site program for adults coming from impacted adult homes. Some support services are provided and class members have access to the Agency's 24 crisis service.**

**PRIMARY RESPONSIBILITY**:
Be an exemplary role model to workers and class members on a daily basis. Conduct oneself in a professional manner at all times.

**TASKS & RESPONSIBILITIES**:

- Coordinate and manage a caseload of approximately 12-18 class members who are adults with mental illness living in community care housing.
- Provide at least two monthly face-to-face visit in each class members’ apartment observing and documenting changes in the mental status, use of the apartment, safety concerns and need for repairs.
- Provide clinical services such as assessment, counseling, link to and coordination of services and advocacy as need.
- Provide hands-on training and assistance of essential independent apartment living skills such as medication management, cooking, shopping and budgeting on an as needed basis.
- Provide orientation to each class member to assist in engaging with the community.
- Assist each class member with obtaining/maintaining entitlements.
- Develop client centered service plans with class members’ input.
- Inform the Program Director of issues endangering the well-being, safety, health or mental status of all Adult Home program residents.
- Maintain ongoing contact with collaterals, treatment teams and other service providers.
- Escort class members, on an as needed basis, to appointments in the community.
- Escort class members to emergency rooms when indicated, which may require mandatory overtime.
- Attend bi-weekly clinical meetings with the Program Director.
- Maintain accurate and current records in the AWARDS database at all times.
- Accurately calculate each class members’ portion of the rent annually and as their income changes according to OMH guidelines.
- Assist class members in communicating with landlords and management companies as advocacy training.
- Participate in pre
- and post
- class member transition calls; communicate and meet with (as needed) health home care managers to assist in the care and coordination of class member services.
- Update arrival and departure time in ADP on a daily basis.
- Participate in annual sexual harassment training.
