Registered Nurse for Street Medicine

We seek adventurous, compassionate people to join our team in providing street medicine and primary care to the underserved. Street medicine is defined as a set of health and social services delivered directly to individuals experiencing unsheltered homelessness on their own terms and in their own lived environment. It is a harm reduction tool meant to build relationships and deliver medically necessary services in order for individuals to stay healthier an be able to avoid unnecessary Emergency Department visits and hospitalizations.
Our street medicines teams deliver care in the field and help remove as many barriers as possible between our clients and the care they need. It is a fantastic way to give back and make a difference, by helping those who need it most.
Our clinics and street medicine teams never turn anyone away, regardless of insurance status or ability to pay. We are passionate about helping our community and are looking for others who share this mentality and are committed to this goal. Our clinics are based in San Diego, but our street medicine teams provide care to individuals experiencing homelessness in areas of greatest need. The Imperial County team works delivering care in El Centro, Brawley, Calexico, and Niland/ Slab City.
The RN will lead a multidisciplinary team of community health workers, and we are looking for someone motivated, responsible, and who enjoys working closely with others. Duties include traveling with the team to the above noted locations, evaluating and examining patients, dispensing over-the-counter medications, providing health education, providing basic wound care and supplies, and assisting with case management. A Physician or NP will be available for tele-medicine backup if a patient needs a prescription or referral to an office-based provider. The Physician or NP will also work on site with the team helping see patients in person 2 days per month. The work is unique and extremely rewarding, and it is a real chance to make a difference in our community and individuals experiencing challenging circumstances.
**Shift and Schedule**
**The team works two days a week, and this can be Mondays, Tuesdays, or Wednesdays.**
**Hours are seasonal and in summer run approximately 6am-2pm, and in shoulder seasons and winter 7:30 am-5:30 pm, depending on patient volumes. This includes time spent in the office charting and documenting at the end of the day.**

**DESCRIPTION OF DUTIES**
The Registered Nurse will be the leader of a multidisciplinary street medicine team of community health workers. We are looking for someone with good leadership skills and who can work well with a team to provide quality, community-based, client-centered care. Duties will include traveling with the team, evaluating and examining patients, providing health education, providing wound care and supplies, dispensing OTC meds, assisting with case management, and contacting the MD/NP remotely when patients need prescriptions, or referral to office-based providers or specialists as needed.
**SUPERVISOR**: Director of Operations / Medical Director/ Street Medicine Supervisor
**REQUIREMENTS**:
- Must have graduated from an accredited Registered Nurse program.
- Ability to manage patient load.
- Relationship-building solid skills to interact with clients, referring physicians, and administrative staff.
- Comfort with and willingness to facilitate the transfer of correspondence between the patient and the patient care team.
- Possess the ability to determine and address the needs of clientele proactively.
**RESPONSIBILITIES**
- Provide nursing evaluation and assessment of patients presenting to the team for care.
- Collaborate with other health care providers, community health workers, and community resources to make appropriate patient and family referrals.
- Identify patients outside the RN's scope of practice and refer them to the Clinic physician/NP or a specialist as appropriate. (If urgent attention is required, the patient will be sent to the hospital emergency department.)
- Act as a consultant with healthcare team members in counseling and teaching selected patients and their families.
- Work in coordinating disease-prevention and health-maintenance programs within the community served by the Clinic.
- Note in the patient's chart any consultation with physician staff regarding the delivery of patient care, indicating the name of the physician consulted in the case.
- Deliver patient care per the Whole Person Care Clinic's Policies & Procedures Manual and Quality Assurance Program standards.
- Ensure complete and proper documentation of all patient care and instruction in patient medical records.
- Precepts graduate and undergraduate students as needed
**Job Type**: Part-time
Pay: $65.00 per hour
Expected hours: 16 - 20 per week
