**Job Purpose**: This professional position is responsible for completing the Adult Long-Term Care Functional Screens (LTCFS) for participants in the IRIS programs served by FPCC. The role of the Eligibility Screener (ES) is to complete annual rescreens and change in condition screens using the Adult LTCFS tool. This position requires travel to all regions served by FPCC ICA.

**Essential Job Functions**:

- Responsible for conducting home visits, assessing participants, and completing Adult LTCFSs as required and outlined in the Wisconsin Adult LTCFS instructions.
- Verifies diagnosis information with physicians and/or SSA when appropriate.
- Complies with documentation and program regulations outlined by the Department of Health Services (DHS).
- Attends and participates in regularly scheduled phone conferences and virtual or face-to-face team meetings as required by the Functional Screen Liaison and Director.
- Responsible for identifying all screens that need to be completed 2 months prior to due date and meets monthly screening quota.
- Attends all DHS, CFI, and FPCC required orientations and trainings.
- Works in partnership with the quality department to develop appropriate measures in support of program outcomes.

**Required Technical Skills, Experience, Education, and Credentials**:

- Requires a bachelors degree in social work, psychology, human services, counseling, nursing, special education or a closely related field.
- One year work experience related to the delivery of social services to the target groups served (intellectual disabilities, physical disabilities and frail elders).
- Must pass the Wisconsin Adult Long-term Care Functional Screen certification modules (80% or higher on each module) within first month of employment and maintain certification as a screener by passing all biannual Continuing Skills Tests (CST).
- Drives for business and requires auto insurance: Access to a personal vehicle, a valid Wisconsin driver's license, and automobile insurance with minimum limits of $50,000/$100,000 bodily injury and $10,000 property damage.
- Must be willing to travel to all counties within First Persons service regions to screen participants and determine functional eligibility.
- Ability to work effectively and cooperatively as a member of the team or work autonomously.
- Strong customer service skills.
- Good organizational and time management skills.
- Excellent assessment and communication skills (verbal and written).
- Comfortable with face-to-face conversations with staff, participants, family and
- caregivers.
- Demonstrated computer and software skills required. Proficient with Microsoft Office suite, Outlook, and Teams. Proficiency with using smart phones.

**Work Environment and Physical Requirements**

- Travel outside to conduct home visits that may or may not meet general public standards of cleanliness and safety.
- Occasionally ascends/descends stairs at various client residences to complete home visits.
- Navigate inside the participants residence and must be able to make observations of the participant and home environment during home visits.
