**Reports To: Program Director**

**Shifts: 5am-1:30pm**

**Summary**:

- The Intake Counselor is responsible for conducting the initial admission assessment for each individual client using the American Society of Addiction Medicine (ASAM) screening process. Intake counselors are responsible for determining an appropriate level of care based on the medical necessity of each individual. Intake counselor’s responsibilities also include completing a variety of risk assessments, Cal-OMS admission, and the ability to diagnose a Substance Use Disorder. The entire admission will be conducted utilizing the (EHR) Specialty Substance Use Disorder System (SAGE).

**Essential Areas of Responsibilities**:

- Complete initial assessment utilizing ASAM screening tool
- Complete Columbia Suicide Risk Assessment on each individual client
- Complete Trauma/Risk Assessments
- Complete Financial Eligibility
- Complete Cal-OMS admission
- Complete SUD diagnosis
- Determine a specific level of care based on each individual independently
- Usage of SAGE in every admission
- Ability to understand Medi-Cal and Other Health Insurance
- Complete Urine Drug Screening and Pregnancy Test
- Provide excellent customer service to potential clients and their family members
- Ability to successfully prioritize in a fast-paced working environment
- Adhere to company HIPAA and confidentiality policies
- Be consciously aware of his/her own scope of practice
- All other duties as assigned

**Knowledge, Experience & Skills**:

- Certified AOD
- Must have 2 years of experience within the chemical dependency field working over the phone or face to face with clients.
- CCAPP or related certification is a plus
- Excellent computer based and telephonic skills - Salesforce/SAGE experience is a plus
- Ability to prioritize and multitask
- Excellent interpersonal and communication skills with a strong client focus

**Physical Demands**:
