**TOUR 2 (9am-5pm)**
**PUPROSE OF POSITION**:
To ensure that all eligible patients receive HRA 2010E packages for housing placement. To ensure social work staff under area of responsibility are carrying out all job responsibilities appropriately.
**AREAS OF RESPONSIBILITIES**:
Social Work Staff located at the five CHAT/HRA sites
**SUMMARY OF DUTIES AND RESPONSIBILITIES**:
- Prepares psychosocial evaluations of clients.
- Will assess clients off site as necessary.
- Has ability to work remotely at least two days per week.
- Provides concrete services and referrals as necessary.
- Maintains statistical data regarding interventions using Microsoft Excel and Microsoft Access Programs.
- Documents all interventions and activities in the patient’s electronic record in accordance with departmental policies.
- Attends necessary Social Work and Comprehensive Health Assessment Team meetings
- Develop schedules and programmatic time tables for the functioning of all five sites.
- Assists in the preparation of budget estimates and controls to ensure that expenses are contained within the budget.
- Assists in the development of staff role and responsibilities.
- Assists in the enhancement of staff clinical skills.
- Responsible for the clinical and administrative supervision of staff.
- Responsible for staff evaluation and development.
- Serves as a member of the interdisciplinary team to assess housing needs of persons with HIV related illnesses.
- Liaise with HRA/HASA/HHC Administrators in matters pertinent to CHAT
- Performs other related functions as determined by supervisor.
- Reports to Social Worker Level V.
**Minimum Qualifications**:
For Assignment to Level III
1. Valid license as an LCSW issued by the NYSED.
