Families First is looking for an individual to join our team as a Integrated Health Home RN

**This position will serve the city of Dubuque in Dubuque county and surrounding areas in Iowa; regular travel is required.**

**Responsibilities**:

- Demonstrate professionalism as an agency representative when partnering with DHS and other community partners
- Demonstrates a sensitivity to service populations’ cultural and SES characteristics
- Synthesize information from treatment records, risk stratification, and other sources to assess program enrollees. Applies evidence-based clinical guidelines for behavioral and physical health issues in addition knowledge of care coordination techniques in assessing the needs of recipients and determining interventions to address those gaps
- Collaborate with IHH team and treating providers in developing and implementing care coordination plan to address treatment barriers for each high-risk member
- Monitor the high-risk care coordination plan and facilitate care and communication amongst treating behavioral and physical providers
- Develop symptom response and crisis plans, ensuring more appropriate use of physician visits rather than emergency room and hospital admissions
- Records and maintains documentation in EMR and Portals.
- Maintain an active workload in accordance with IHH performance standards, striving to achieve benchmark goals
- Participate actively in the Quality Improvement Committee outcomes and objectives
- Promote patient’s health and ensure that all personal wellness goals are included in person-centered care management plans
- Provide health education to patients on preventing and managing chronic conditions using evidence-based sources
- Provide self-management support and development of self-management plans and/or relapse prevention plans so that members can attain personal health goals
- Assist with the collection of data for special projects and presentations for IHH
- Assess patients for any health care gaps and assist patients in addressing those gaps to improve healthcare outcomes.
- Conduct home visits as needed
- Support day to day collaboration between physical health and behavioral health providers
- Understand and Implement Population Management strategies
- Other duties as assigned
- Demonstrates a working knowledge of mental health diagnoses, symptoms, and effective treatment interventions
- Utilizes a holistic, strengths-based approach to working with clients
- Perform other duties as assigned

**Qualifications**
- Education requirements:

- A Bachelor’s Degree from an accredited business or technical college or university with emphasis in Biology, Human Services, or a closely related field; and

Work Experience: Two (2) years’ experience and knowledge of working with children with mental illnesses

An equivalent combination of education, training and experience that provides the knowledge, skills and abilities necessary to perform the essential functions of the position.

Required licenses, registrations and certifications: Possession of a valid Driver’s license within the State of Residence. State of Iowa Registered Nurse (RN) license.

Pay: $50,000.00 - $70,000.00 per year
