The Health Partnerships Liaison furthers the mission of the Houston Food Bank (HFB) and is responsible for maintaining operations of all day-to-day health programs and project implementation across HFB’s 18-county service area. This position is responsible for providing ongoing support and technical training, issue resolution, problem-solving, community engagement, customer service, and compliance visits for health partners. This position maintains relationships with health partners, and various community collaborators and provides exceptional customer service and support to all internal and external stakeholders. This position provides consistent first point of contact and troubleshooting with health-aligned partners.

**ESSENTIAL JOB FUNCTIONS**:

- Establishes and maintains relationships with health partner organizations and pantries that implement health-related programming (i.e., food prescriptions, food pharmacies, medical pantries, healthcare partner-funded school pantries, and hospital food insecurity mitigation programs).
- Provides ongoing programmatic support to partners and conducts regular site visits and evaluations to ensure partners are implementing health programming with fidelity.
- Develops, coordinates, and delivers health program onboarding orientations and training to partner organization staff at regular intervals.
- Manages the process of data collection and sharing between HFB and health partners to demonstrate key performance indicators.
- Makes suggestions for refining HFB’s health program interventions, including identifying and sourcing appropriate produce and food products for health program participants, coordinating logistics of the Healthy Market Trailer, and determining referral systems for ensuring participants redeem produce prescriptions.
- Monitors health program participants’ redemption rates and assesses and synthesizes data and provides detailed reports.
- Maintains records of health programming processes and procedures for each partner.
- Analyzes data to identify opportunities to strengthen the program and present findings to leadership.
- Coordinates activities associated with health collaborative initiatives.
- Maintains timely correspondence with health partners regarding health programming; refers other calls to appropriate HFB personnel.
- Provides information and assistance to partners on the various electronic platforms (Link2Feed, Partner Portal, Form Assembly).
- Supports the implementation of grant-funded activities, nutrition policy, and Healthy Pantry Project with identified partners and pantries.
- Maintains knowledge of current operational processes at identified pantries and identifies opportunities to provide support as needed.
- Works with appropriate HFB staff to address partner issues to resolution.
- Ensures that departmental documents are completed accurately, timely, and stored properly.
- Updates departmental data and information on all digital platforms.
- Acquires, compiles, and updates departmental reports.
- Provides support to other team members with relevant tasks and contributes to problem resolution on different projects.
- Travels throughout HFB’s service area to promote and recruit healthcare and other HFB programs.
- Attends and actively participates in relevant community partnerships groups and meetings.

**SUPERVISORY RESPONSIBILITIES**:
This position has no supervisory responsibilities.

**QUALIFICATIONS**:
**Education/Experience**:

- Bachelor's degree from an accredited four-year college or University in Public Health, Nutrition, Social Work, Social Science, or a closely related field
- 1-2 years of community related experience
- Bi-lingual English/Spanish preferred

**Certificates, Licenses, and Registrations**:

- Must have reliable transportation, a valid driver’s license, and insurance.

**Special Knowledge/Skills/Abilities**:

- Proficient in Microsoft Office Suite, including but not limited to Excel, Word, PowerPoint, and Outlook
- In-depth understanding of the intersectionality between Social Determinants of Health, food insecurity, proper nutrition, and health outcomes
- Ability to work with a diverse population, including members of the community
- Excellent written and verbal communication skills
