**Job description**

Job Duties:

- **Direct Care and Support**:

- Assist with activities of daily living such as bathing, dressing, grooming, and meal preparation.
- Professional Crisis Management ( Behavior Focus Facility)
- Administer medications according to prescribed dosages and schedules.
- Monitor and document vital signs like blood pressure and heart rate.
- Ensure the safety and well-being of residents by adhering to proper protocols and procedures.
- **Case Management**:

- Assist with case management by coordinating appointments and communicating with healthcare professionals.
- **Advocacy and Promotion**:

- Advocate for resident rights and promote their needs and preferences.
- Maintain a clean and organized living environment for residents.

**Requirements**:

- **Experience and Knowledge**:

- Preferred experience working with individuals with developmental disabilities or in a healthcare setting.
- Knowledge of dementia care, hospice care, and working with people with developmental disabilities is advantageous.
- **Skills**:

- Ability to administer medication accurately and safely.
- Strong conflict management skills to handle challenging situations and behaviors effectively.
- Familiarity with resident rights and the ability to advocate for them.
- Basic knowledge of case management principles and practices.
- **Certifications**:

- First aid certification is required; CPR certification is required; PCM certification is required; APD Certs
- Background Check and Local Law is required.
- **Communication and Attention to Detail**:

- Ability to communicate effectively with residents, families, and healthcare professionals.
- Attention to detail and ability to follow instructions accurately.

**Salary**: $15.00 - $18.00 per hour

Supplemental pay types:
