The Lead Registered Nurse (LRN), a key member of the Medical Team, will lead a clinical care pod while performing both supervisory and clinical nursing duties, which include both administrative and clinical responsibilities. Under the direction of, and in coordination with, the Director of clinical Operations, Chief Medical Officer and Site Director; the (LRN) ensures the provision of high quality, well-coordinated medical services in a clinical care pod model. The LRN ensures the Medical Department remains complaint with regulatory agencies, training and on-boarding practices and provides case management.

The Native American Health Center is an accredited institution and adheres to the standards of excellence set forth by the Accreditation Association of Ambulatory Health Care (AAAHC) and the Commission of Dental Accreditation (CODA).

**DUTIES AND RESPONSIBILITIES**
**Clinical Floor Duties**
1. Perform clinical nursing duties with high integrity, quality and under the clinical supervision of the Medical leadership and in coordination with the clinical care pods.
2. Provide case management to NAHC members including but not limited to those with chronic health conditions such as HIV, HEPC and diabetes. Case management
duties include, but are not limited to, executing the medical orders of the physician, monitoring labs, calling in prescription orders and coordinating any necessary prior authorizations and insurance issues, advocating for treatment adherence, and diligently documenting this work in the medical chart.
3. Responsible for the acquisition and stocking of all medications dispensed from the clinic.
4. Ensure pharmacy medication logs, refrigerators and room temperatures are maintained.
5. Supervise and administer vaccines and medications, as required, during office visits.
6. Assist and facilitate patients in obtaining medical services, including scheduling patients as appropriate, triaging patients, and assessing patients who walk
- in to the clinic.
7. The LRN will ensure patients are seen on a timely basis, identify and communicate barriers to efficient flow and identify possible solutions including, but not to limited to leading interdepartmental meetings and trainings.
**Patient Care Duties**
1. During Visit, is responsible for the following:
a. Assist in managing Medical emergencies / Code Blue.
b. Patient Assessment and Intake/triage.
c. Assist in performing; or performs procedures when requested by Medical Provider. Examples include, but are not limited to, Diabetic Retinal Screening, Wound Care, Pregnancy test, U-tox screens, TB tests, EKG, Snellen etc.
d. Administer IM, ID, SC, PO, oxygen and nebulized medications as ordered by the provider.
e. Provide Patient education as requested by Provider. Examples include, but are not limited to:
i. Medication and Diagnosis
ii. Smoking Cessation
iii. Contraception use
iv. Blood Pressure check and education for home monitoring.
v. Diabetes Mellitus management education (Medications and Glucometer use).
vi. Inhaler and Spacer use.
vii. In collaboration with a Provider, run Medical Group Visits as directed by Medical Leadership.
2. Responsible for post-visit activities and Case Management, including:
a. Intensive Case Management
b. Transition of Care Management
c. Addressing Phone calls from external Physician’s office or Emergency Department (Clinical only)
d. Respond to urgent phone calls from members.
e. Submits prior authorizations
f. Coordinate referrals, as needed
g. Complete all standing orders, as clinically appropriate
**Regulatory and Compliance Duties**
1. Maintain Children’s Health and Disability Prevention (CDHP), Vaccines for children VFC), CAIRS program certifications, and in providing annual required educational trainings by CHDP, VFC, CAIRS and others.
2. Coordinate Departmental Audits, ensuring audit readiness for AAAHC, VFC, IHS, health plans, etc. Escorts auditor and assist with audit.
3. Coordinate Quality Assurance checks on health records and referral information to ensure that records are audit ready (i.e., by VFC, CHDP, Managed Care Medi-Cal, Accreditation Association for Ambulatory Health Care, Patient Centered Medical Home, etc.)
4. Ensure all logs, forms, records, and charts are complete, thorough, accurate and stored according to HIPAA compliance.
5. Coordinate regular site visits for Pharmacy Consultant for 340B Audit readiness.
6. Support implementing recommendations and corrective actions, as necessary.
7. Develop, update and implement Policies and Procedures and workflows.
8. As needed, review and update clinic forms to maintain compliance with relevant local, state, and federal law.
9. Train all clinic staff in new (and old) policies and procedures, as needed.
10. Oversees all aspects of the management of clinic facilities and equipment including maintenance and ordering of clinic inventory, medications and supplies.
11. Supervises the immunization project to include planning, ordering, training, report writi
