This is a Full-Time Hybrid position. This flexible position offers autonomy while utilizing your nursing assessment, care planning and case management skills. The WeCare 365 Adult Foster Care program is licensed by the State of Massachusetts to provide much needed support to our members and those that are caring for them. The AFC program provides clinical support and a stipend to caregivers for the valuable work they do. We are committed to caring 365 days a year with care plans carefully tailored for member needs through a team approach for holistic, wrap-around services. **EXPERIENCE**:
- Registered Nurse in Massachusetts
- Bachelor’s degree
- Experience with Minimum Data Set (MDS) assessment tool a plus
- Care Management experience
- Monthly, Bi-Monthly visits (in person and/or telephonic)
- Strong communication skills
- Medication Management
- Medical documentation and transcription
- Experience with databases and Microsoft Office
- Caregiver Training and Education
- Must be able to travel and have a reliable vehicle
**Job Type**: Part-time
Pay: $40,000.00 - $80,000.00 per year
