**Care Designer / Director of Nursing (Job Description)**

**TITLE OF IMMEDIATE SUPERVISOR**: Administrator / Chief Operating Officer

**OVERVIEW**

The Care designer / Director of Nursing is a multi-faceted position responsible for planning, directing, and evaluating client and staff care, implementing standards of care, and developing staff personnel. This role provides leadership and direction in all aspects of client care, including health education, counseling, and follow-up care. The individual will drive business results through client experience and caregiver management, ensuring compliance with national, state, or provincial legislative requirements.

The Care Designer/Director of Nursing is responsible for partnering with the Community Engagement Specialist to drive business growth by building and maintaining community partnerships. The position is also responsible for partnering with the People Experience Coordinator to ensure adequate caregiver staffing levels.

**ESSENTIAL DUTIES & RESPONSIBILITIES**
- **Client Care Management**_
- Implement administrative and client care policies in compliance with regulations.
- Conduct Caring Consults, assessing clients' physical, emotional, and environmental needs, and advising on client care.
- Oversee client care management processes, including intake, case setup, customer service, and service provision.
- Ensure skilled nursing and therapeutic services are under proper supervision.
- Serve on the Professional Advisory Committee and periodically review policies.
- Develop and maintain individualized care plans and update as required.
- Monitor risk issues, analyze trends, and adapt care as needed.
- Achieve/exceed Client Net Promoter Scores and promptly investigate issues.
- **Staff Management & Development**_
- Conduct employee selection processes, including recruitment, hiring, and onboarding.
- Supervise, schedule, and manage overall performance of staff.
- Ensure staff development through orientation, in-service education, continuing education, and evaluation.
- Verify credentials and qualifications for caregiving personnel.
- Achieve/exceed Employee Net Promoter Scores and promptly investigate issues.
- **Quality Assurance/Quality Improvement/Risk Management**_
- Participate in the review, analysis, and appraisal of program effectiveness.
- Direct and monitor performance improvement activities.
- Oversee quality assurance and utilization review activities, ensuring timely implementation of corrective actions.
- Manage organizational risk, ensuring compliance with legislative requirements.
- **Business Development**_
- Promote company services across various marketing avenues.
- Develop and maintain partnerships with healthcare providers and clients to drive growth.
- Attend networking events and meetings to represent and promote the company.
- Actively develop relationships with hospitals, physicians, SNFs, and ALFs to enhance sales growth.
- **Administration**_
- Maintain accurate documentation practices for client and employee files.
- Review and update records quarterly to ensure accuracy and completeness.
- Ensure compliance with health, employment, and safety regulations.
- Approve all visits and premiums in preparation for payroll and billing cycles.
- **Scheduling Collaboration**_
- Ensure accurate and timely client and employee files are handed off to the Care Services team.
- Liaise with the Care Services team to communicate changes in client requirements and caregiver assignments.
- Ensure client coverage, especially for urgent care or caregiver cancellations.

**QUALIFICATIONS**

**Education Requirements**
- Registered nurse with current unencumbered licensure to practice professional nursing.
- CPR and First Aid Certification

**Experience Requirements**
- Minimum 3 years of nursing or nursing supervision experience in home care or home healthcare organization.
- Strong computer skills, proficient in Google-Suite.
- Exceptional written and verbal communication skills.
- Strong organizational, planning, time management, and multitasking skills.
- Proven ability to build quality and implement improvements.
- Excellent interpersonal skills and customer service background.
- A Sales & Marketing background and experience is strongly preferred.
- 1 - 3 years of direct employee supervision strongly preferred.
