Mid Shore Behavioral Health, Inc., a 501(c)(3) nonprofit organization located in Easton, MD, is seeking Continuum of Care (CoC) Housing Specialist to provide case management, community engagement and outreach services in the mid-shore region. This position reports to the office located at 28578 Mary’s Court, Suite 1, Monday - Friday, 8:00 - 4:30. Frequent and routine travel to off-site locations is required.

This position requires frequent, routine driving, walking, and climbing steps, in order to perform outreach and case management responsibilities. These responsibilities are conducted in a variety of settings to include tent cities, encampments, and uninhabitable places such as abandoned homes, homes without utilities and other sites that may present unanticipated obstacles to safety. Excellent judgement is required for this position in order to problem solve and make decisions that support the health and safety of all involved. This role requires high proficiency in active listening and de-escalation techniques. There is a high level of accountability to following risk management procedures promoting safety through compliance to agency protocols. Time management and attention to detail is required to follow up with paperwork and documentation necessary to sustain funding and effective case management.

**Specific duties and responsibilities include but are not limited to**:

- Provide person centered intensive and general case management services.
- Assist participants in moving toward self-sufficiency.
- Assist new participants with signing and understanding a lease, tenant responsibilities, landlord responsibilities, etc.
- Work closely with the PATH Outreach Case worker to ensure the neediest homeless individuals and families are placed in programs as quickly as possible.
- Participate in the Roundtable on Homelessness.

All employees participate in routine, facilitated diversity, equity, and inclusion (DEI) meetings, trainings, and discussions to help raise awareness of systemic and community issues, as well as strengthen a culture within MSBH that fulfills the promise of a fully inclusive and trauma-informed work environment.

**Required Skills**: Microsoft Office, Online Database Systems, experience in direct service/support with consumers, excellent communication and documentation skills, time management and organizational skills. Desired skills: knowledge of housing and homeless issues in the mid-shore region.
