Job Summary:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing support, guidance, and resources to individuals and families in need. Your role will involve assessing client needs, developing care plans, coordinating services, and advocating for clients within the community. If you have a passion for helping others and possess strong interpersonal skills, we encourage you to apply.

**Responsibilities**:

- Conduct comprehensive assessments to determine client needs and develop individualized care plans
- Provide crisis intervention and support to clients experiencing difficult situations
- Collaborate with healthcare professionals, social workers, and other service providers to coordinate client care
- Advocate for clients' rights and access to necessary resources
- Monitor client progress and make adjustments to care plans as needed
- Provide behavioral health support, and assistance with accessing mental health services
- Provide guidance on accessing Medicare and Medicaid benefits and other healthcare resources
- Offer mentoring and coaching to help clients develop life skills and achieve their goals
- Maintain accurate and up-to-date documentation of client interactions and progress

Qualifications:

- Bachelor's degree in social work or related field (Master's degree preferred)
- Previous experience in case management or related field
- Knowledge of crisis management techniques and intervention strategies
- Strong communication skills, both written and verbal
- Ability to work collaboratively with a diverse range of individuals and organizations
- Excellent organizational skills with the ability to manage multiple tasks simultaneously

Note: All positions at our organization are paid positions.

Pay: $50,000.00 - $57,000.00 per year
