**Program Overview**:
VOA Illinois Supportive Services for Veteran Families (SSVF) is a comprehensive program for Veterans who are struggling in crisis or at risk of serious instability in their lives. The program provides Veterans with the support, resources and hope they need to successfully win “the war at home” and achieve their full potential in all areas of their life, by working on a personalized service plan with our Veteran Services team.

The SSVF Program reaches out to Veterans wherever they are in their journey towards healing and stability and offers a full continuum of supportive services and interventions that are individually tailored to meet the unique needs of each Veteran. At its core, it is strength-based and works to build on the inherent strengths and assets that exist within each Veteran we serve. The program also uses a very relationship-driven approach to helping Veterans rebuild and transform their lives - relationships with other Veterans and with caring, committed staff.

The National SSVF Program Office introduced the Health Care Navigator position in 2020 to bridge the gap in services for individuals in need. The HCN is tasked with connecting veterans to appropriate benefits and medical services including VA Service Connected and Non-Service connected benefits, SSI/SSDI, Medicaid/Medicare, VA Healthcare, community based health care, etc..

**Position Type**:This is a Full-time, in-office position and requires a 40-hour week on average. This is a Monday through Friday, 9 to 5 position. There may be opportunities to attend weekend or after hours meetings or events as needed.

**Base Salary Range**: $50,000-$57,000 (commensurate with education and experience).

**Environment**:Physical demands include sedentary work, operating a computer and other office productivity machinery with frequent communication with clients and colleagues of VOA Illinois and others involved in the care and custody of clients. The individual hired must be able to move, traverse and travel in the community in a vehicle, assist clients in and out of a vehicle if necessary and transport clients in the community. The individual is expected to travel between office sites including the headquarters office in the South Loop, our Englewood offices, and our Joliet offices as needed.

- Engage with trauma-informed care approach and practice principles.
- Recognize and display ability to de-escalate and resolve conflict as necessary.
