Our mission is to provide critical life assistance and professional mental health services with an emphasis on economically disadvantaged, marginalized youth, and senior adults in the greater South Florida metropolitan area. We provide high quality, culturally relevant care for all with an emphasis on those in financial need, minorities, youth, seniors, HIV+ persons, and those with life-challenging physical or emotional conditions.

YOUR CONTRIBUTION TO OUR SUCCESS:
**Position Requirements**:The Community Outreach Coordinator should have extensive experience working with lesbian, gay, bisexual, transgender, and gender variant adults. Good teamwork and communication skills required as well as experience with HIV+ clients, substance abuse and trauma.
**Position Description**:
- Weekly and monthly billing for journaling program cohorts, gathering and entering data as required.
- Addressing corrective action plans from United Way as necessary, which may include creating new forms, overseeing chart uniformity and quarterly chart reviews.
- Gathering data (statistical, cohort feedback and program success rates) for yearly performance report to United Way.
- Engage in community outreach to increase program participation, including attending job and resource fairs to inform the community of SunServe’s Interactive Journaling Program, Prevention Program, and other services.
- Identify new community partners for referrals to SunServe Services.
- Ensuring journals are ordered and available for new cohorts.
- Reviewing new intakes with intake manager to assess criteria for IJ Prevention eligibility.
- Documents and reports client services in a manner that complies with agency and Florida professional standards.
- Provide crisis intervention as needed. Other duties as assigned.
- Establish a trans-focused interactive journaling group.
- Develop HIV testing programming.
**Knowledge, Skills & Abilities**:
- Considerable knowledge of community resources throughout Broward County.
- Willingness to pursue further education and/or training.
- Strong communication skills.
- Ability to establish and maintain effective and collaborative working relationships with other
- Experience working with diverse populations, various backgrounds, and cultures.
- This role requires work outside of regular office hours, such as evenings and weekends.
**Minimum Qualifications**:
- Bachelor’s Degree or equivalent work experience
