**Job Scope**: Transition specialists coordinate and provide services to assist adults to transition out of long-term care facilities and into community settings. Services are provided on-site at area nursing facilities, community areas, and in client homes at flexible times to meet each client’s needs. This position will operate primarily in the Lake County area with weekly required in-office hours in Arlington Heights, IL. Tasks include locating housing, viewing apartments, purchasing items to setup a new apartment, locating transportation, obtaining identifying documentation, setting up aftercare services, and coordinating discharge activities. Work closely with clinical care manager staff to ensure all client needs are met. Must have a Bachelor’s Degree and have a valid Illinois Driver’s License. Preferred 1 year of experience working with adults with serious mental illness and/or adults with other complex physical health and mobility needs. Preferred knowledge of Olmstead Colbert and/or Williams Consent Decrees.
**Clinical Responsibilities**
- Provide transition services for Colbert and Williams Class Members moving from a nursing facility or SMHRF (Specialized Mental Health Rehabilitation Facility) by locating housing, completing transition activities, linking with medical providers, and accessing community resources
- In coordination with Clinical Care Manager, participate in care planning, discharge planning, and staff meetings for Colbert and Williams Class Members
- Assist in coordinating discharge planning activities for Williams and Colbert Class Members to ensure a smooth transition by coordinating and consulting with family members, property managers/landlords, managed care coordinators and nursing staff, nursing home staff and medical providers, KYC’s Williams and Colbert Quality Administrator and Clinical Care Managers, and staff at the Department of Aging, Department of Mental Health, and various housing authorities
- Work with class member to identify and budget all items needed for smooth transition, and assist with shopping and apartment setup
- Routinely consult with other KYC teams including employment services, IRG, substance abuse/SUPR, drop-in staff, and psychiatry to address specific client related clinical issues and coordinate care strategies; and other agencies within the targeted transition area
- Promote opportunities for clients to build natural systems of support within their communities through exposure to recreational, spiritual, social, educational, employment, volunteer and civic activities
- Work cooperatively with other KYC staff and outside providers in order to maintain smooth transitions and effective continuity of care when cases are shared
- Provide independent living skills training, supportive therapy, case management, medication training, and community support services
- Facilitate referrals for additional services consistent with assessed needs and client’s desired outcomes
**Administrative and Performance Requirements**
- Work with other members of the Comprehensive Transition team to assess, plan, and participate in provision of services
- Participate in all staff meetings and clinical staff meetings and trainings as directed by supervisor
- Meet and exceed productivity expectation
- Participate in supervision as directed and seek additional clinical and program consultation as needed
- Interpret and communicate, at the service delivery level, KYC policies and procedures
- Complete all clinical documentation and required reports in an accurate and timely manner
- Complete all documentation and forms required to coordinate the transition process for Williams and Colbert Class Members
- Meet agency quality and compliance standards
**Travel**
- The employee is required to travel by automobile to and from various KYC office locations on a weekly basis, as well as frequent client visits at their homes, area hospitals and nursing home facilities in the Lake County area, and occasional meetings/trainings in and around the Chicagoland area.
