QUALIFICATIONS
- Must be a Registered Nurse or Licensed Professional Nurse in good standing and currently licensed by the

State.
- Minimal (2) years previous MDS/care plan and Long Term Care experience.
- Must be able to relate positive to residents, families, and government regulators, and to work cooperatively

with other associates at all levels.
- Must have experience in performance management and effective leadership.
- Must have ability to complete multiple projects.
- Must be able to travel, including overnight stays.
- Must be capable of maintaining regular attendance.
- Must meet all local health regulations, and pass post-employment physical exam if required. This requirement

also includes drug screening, criminal background investigation and reference inquiry.
- Must be capable of performing the Essential Job Functions of this job, with or without reasonable

accommodation.

ESSENTIAL JOB FUNCTIONS

- Oversee and coordinate the development and completion of the resident assessment (MDS) in accordance with

current Federal and State rules, regulations, and guidelines that govern the resident assessment
- Assist in the development of a comprehensive resident assessment and care plan for each resident
- Disseminate any new or updated materials involving RAI process.
- Facilitate the involvement of appropriate health professional needed to improve or maintain the resident’s

functional abilities at the highest practicable level.
- Create an opportunity for family participation in the care planning process.
- Ensure the face validity of all Minimum Data Sets before electronic submission.
- Participate in and oversee the timely electronic submission of al MDSs.
- Review the validation report and ensure that appropriate action is taken.
- Serve on, participate in, and attend various other committees of the facility as required, and as appointed by

the DON or Regional Director.
- Conduct a monthly review of a random sample of a minimum of 2% of the MDS to ensure accuracy and

internal consistency with the medical record.
- Work collaboratively with the product champions to ensure that all new nursing associates are knowledgeable

of the RAI process and receive the appropriate training.
- Communicate with Business Office Manager and Administrator on a regular basis regarding the case mix

scores and how they impact reimbursement.
- Collaborate with product champions to ensure training of the clinical software to new associates.
- Comply with all Company policies related to safety and infection control procedures including the proper use

of mechanical lifts, gait belts, and personal protective back supports.
