Therapeutic Mentor

he Therapeutic Mentoring (TM) program is a service provided to youth under the age of 21 in their home and in other community settings such as school, after school programs, or other community-based environments. TM services include supporting, coaching, modeling, and teaching age-appropriate behaviors, communication, social, problem-solving, and conflict resolution skills, as well as relating appropriately to other youth and adults. The work of the TM is designed to complement and build upon the work being done with the youth by the hub provider (Individual OP Therapy, In Home Therapy, or Intensive Care Coordination).
**Essential Job Responsibilities**:
The Therapeutic Mentor:
- Works one-on-one with the youth in their home and community, helping them to develop and practice the skills needed to achieve their goals as outlined by the hub provider (OP, IHT, or ICC).
- Works collaboratively with the hub provider in determining the goals, activities, and frequency of TM services.
- Maintains at least weekly contact with the hub provider.
- Meets at least 3-4 hours per week (or more as appropriate) with the youth to implement skills-development strategies in a home or community-based setting (outreach).
- Completes necessary documentation including, but not limited to, daily progress notes, risk assessment, lesson plan, and discharge plan within required timeframes.
**Job Type**: Part-time
Pay: $30.00 - $32.00 per hour
Expected hours: No less than 15 per week
