**Responsibilities:

- **

**(Health Data Specialist 2)** is to be a Masters-level professional with experience in public health/behavioural health program management with excellent quantitative analytical skills.

The responsibilities include program management, programmatic and data reporting, managing the flow of information between contracted providers and DMHAS offices, and providing contract and audit support. Other responsibilities include-- evaluates and analyses the quarterly data received from the service agencies; Identifies trends and/or potential best practices; Monitors the performance of the service teams through regular communications, and reviewing quarterly progress reports from the agencies; Perform other tasks(e.g., logístical support for meetings, document production, and technical assistance) and special projects as requested. The staff member will be able to create presentations and slide shows and present the data and the progress of the program initiative at internal meetings as well as external meetings with stakeholders as requested.

Abstract clinical information for entry into internal and external transplant databases as instructed and the data mining of this information as requested
Create tables, queries, forms and reports in Microsoft Access, PowerPoint and Excel as needed
Coordinate and analyse data and clinical status information of patients
Review medical records for data collection and analysis to meet requirements for necessary forms, completes data entry, initiates and monitors compliance and maintains applicable spreadsheets, research forms, and consents
Assist with long-term follow up the process in collaboration with Coordinators, APCÆs, RNÆs
Participate in the organization of quality improvement meeting data including morbidity and mortality, engraftment, positive microbial culture results and continuous resource management
Manage all aspects of workgroup security, maintain data integrity (including running regular tests to ensure data accuracy), maintain data tables, and ensure redundant backup measures are in place
Document all critical work processes with clear policies and procedures
