Case Management and Accountability

Job Description, Responsibilities, and Duties.

Case Managers perform several tasks to achieve the best outcomes for their clients mental, emotional, and physical well-being. These duties usually involve working with their clients directly and often working with other staff and outside resources to achieve the best outcomes for each client. Responsibilities include but are not limited to:

- Creating tailored goal plans for each client to achieve over the course of treatment.
- Helping clients with employment opportunities, career trades, school enrollment, etc.
- Prioritizing the needs of each client.
- Assisting clients with setting up their health, dental, and vision appointments.
- Documenting each client's session with detailed information regarding the session.
- Following up with the continuous improvement of each client mentally, emotionally, and physically.
- Finding current and future resources around the valley that will benefit our present and future clients.
- Building business relationships w/ other prominent and reliable resources for our clients and our team.
- Communicate to Admin on any appointments made during CM sessions.

CM’s will collect the data of client appts and pass the information onto Admin for transportation coordination.

CM will send a weekly update (Monday’s) to House Mgr/night staff on who has food stamps, who does not have food stamps, and how much each client receives monthly.

Each client is to be seen for 30 - 60 minutes.

All client appointments must be reported to the CM by the client during a CM session.

Appointment times are from 9am - 4pm on ONLY:

- Tuesday’s,
- Wednesday’s
- Thursday’s

NO APPOINTMENTS ON MON. or FRI.

ALL appointments have to documented and put on the calendar for Admin to coordinate transportation.

NO LAST-MINUTE APPOINTMENTS.

If the appointment is not on our calendar’s, it does not exist.

Individual documentation is required on each and every client seen by CM and must be turned in at the end of the day to Admin.

DES (food stamps status/expiration) matters are for CM execute.

Pay: $18.00 - $22.00 per hour

- Weekends as needed

**Education**:

- High school or equivalent (required)

**Experience**:

- Microsoft Office: 1 year (required)
- Microsoft Word: 1 year (required)
