ESSENTIAL FUNCTIONS
- Coordinate case management and family reunification services for children.
- Initiate and maintain direct contact with each child and the child’s family while the child is in funder/oversight care in order to efficiently advance the child’s family reunification and release the child from the facility.
- Facilitate the timely release or discharge of children to identified family members or authorized caregiver/custodian and document the provision of services in each child’s case file.
- Conduct and document funder/oversight assessments in accordance with funder/oversight policies and procedures.
- Report significant incidents in accordance with funder/oversight policies and procedures.
- Maintain the client case file and funder/oversight database in accordance with funder/oversight policies and procedures.
- May travel (including out of state) with unaccompanied minor to new placement.
- Models Center’s expectation for sensitivity and competency relating to diversity and cultural awareness.

EDUCATION / EXPERIENCE
- Bachelor’s degree in social work, psychology, sociology, or other relevant behavioral science.
- Minimum one year experience in case management.
- Clean criminal background check.
- English / Spanish bilingual capabilities required.
- Must possess valid Level One Fingerprint Clearance Card or able to obtain one prior to employment
- Must be at least 21 years of age
- Must possess a valid driver’s license with a 5-year clean driving record
