**Rehabilitation Specialist**

The Rehabilitation Specialist (RS) is responsible for providing the clinical services to the clients of New Day New Start Behavioral Health Center and ensuring that the client’s emotional and medical needs are met and maintained in accordance with agency policies and procedures, Commission on Accreditation of Rehabilitation Facilities (CARF) standards, and current federal, state Code of Maryland Regulations (COMAR), and local standards, guidelines, and regulations. The RS also has the responsibility of supervising the direct care workers and ensuring that the clients are receiving the appropriate services as outlined in their

Individual Rehabilitation Plans. The RS also assists with providing the necessary training for direct care workers.

**Qualifications**: The RS must have **o**ne to two (1-2) years of supervised social work experience in a health care setting, working directly with individuals. Experience working with people with mental health disorders preferred.

**Hiring Requirement**:The RS must have a current and valid (in good standing) Maryland certification as a LMSW, LCSW, LCSW-C, LCPC, or LGPC.

**Reports To**:Chief Executive Officer or designee.

**The RS’s primary responsibilities are as follows**:

- Ensures the completion of all client assessments.
- Ensures that each client has a current and comprehensive individualized rehabilitation plan.
- Ensures that all client contact is documented appropriately in the form of session notes.
- Ensures that each client has a current and comprehensive rehabilitation monthly progress report.
- Ensures ongoing communication and coordination of services between the mental health service provider and the rehabilitation service provider.
- Supervises service delivery to ensure the effective implementation of treatment and case management services.
- Supervises the completion of clinical documentation. This includes ensuring documentation is completed in a timely manner and its content is of sufficient quality.

In addition, ensure that the clinical treatment flow is clear, comprehensive, and observable in the client file.
- Supervise the clinical service delivery to ensure effective implementation and execution of clinical interventions and clinical supervision.
- Participates in monitoring the revision and continued development of policy and procedures in collaboration with Chief Executive Officer or designee.
- Participates in facilitating the development and revision of forms and tools utilized to administer rehabilitation services in collaboration with the Chief Executive Officer or designee.
- Facilitates group and individual supervision meetings with direct service workers.
- Facilitates In-Service Training once monthly for direct care workers in collaboration with the Chief Executive Officer or designee.
- Ensures medical compliance is documented on appropriate forms for maintenance of health passport and forward medical information to appropriate parties.
- Ensures compliance and securing medical attention for the overall well being of clients in care.
- Coordinates team meetings to develop rehabilitation plans for clients and ensure implementation of treatment services.
- Documents clients’ progress in monthly treatment summaries.
- Coordinates with therapist and other outside agencies for effective and efficient delivery of services.
- Collaborates with community resources and services to meet clients’ needs.
- Maintains client files with appropriate medical, educational, and treatment records and facilitate correspondence with inter-agency team members.
