**Job Summary**:
Join a collaborative and support team that provides direct case management services to adults with a primary mental health disorder residing in Bergen County who are at-risk or involved in the criminal justice system to divert incarceration and reduce recidivism

**What You Will Do**:

- Assess and evaluate consumers at intake, admission, during ongoing treatment, service delivery, and at discharge or release from incarceration.
- Collaborate with treatment providers to establish comprehensive assessments and service plans to increase tenure outside of the correctional system.
- Conduct outreaches, assess consumers, and communicate the integration of consumer history and information for evaluation
- Create psychological service plans containing relevant, behaviorally measurable goals and realistic time frames for achievement.
- Provide substance abuse services, family consultation, crisis intervention, vocational, educational, employment services, and comprehensive services based on the Risk of Hospitalization.
- Assist consumers in completing appropriate financial and insurance forms
- Monitor team functioning to ensure consumers receive the required care and research and coordinate resources for service provision and training.

**What We Need to See**
- A Master's degree OR Bachelor's degree in Psychology or a related field, plus at least one year of experience working with individuals with severe and persistent mental illness.
- A valid driver's license must be insurable by the agency's automobile insurance.
- Case management experience is preferred.
