**SUMMARY**:
The Beverly Hills Housing Supportive Services (BHHSS) is a 10-year pilot program funded by the city of Beverly Hills. This program will provide permanent supportive housing for chronically unhoused individuals in the city of Beverly Hills, while supporting them to develop life skills, increase self-sufficiency and to integrate into the community. The SCI position This will provide on-site support including (but not limited to) case management and linkage, supportive services, and housing retention services to members utilizing project-based and scattered site housing. Step Up on Second BHHSS program is guided by the principles of the Housing First Model and will utilize Evidence Based Practices including Motivational Interviewing and Harm Reduction models of service delivery.

**DUTIES**: The following reflects essential functions for this job but does not restrict other tasks, which may be assigned:
**Leadership, Administration, Oversight**:

- Liaise with other providers within the agency, as well as outside providers to formulate strength-based strategies that focus on housing retention and general wellness.
- Participate in staff meetings, individual and group supervision, case conferences and training as required.
- Engage and actively participate in any training aimed on how to assist members in effectively engage with behaviors essential for maintaining housing.
- Complete timesheets and request for any time off in a timely manner as outlined in the SUOS employee handbook.

**Direct Service and Support**:

- Work directly with a diverse and marginalized group of about 20-40 participants, experiencing many barriers including homelessness, complex trauma, medical, mental health needs, and substance use.
- Evaluate members to identify social, emotional, and economic factors which may interfere with attaining stability and optimum health.
- Assist members with recertification and completion of any documents required to maintain the unit and facilitate the move in process before and after move in.
- Meet with members face to face at least once a week (more if needed) to engage in completing needed documentation for obtaining housing and engaging in essential independent living behaviors once housed.
- Transport/accompany member to unit searchers, viewing, lease signing, medical appointments and to various agencies.
- Assist Members with developing and learning new skills that lead to housing household management, cleaning, cooking, household upkeep and cleanliness, and non-hoarding behavior.
- Ensure services to members are reflective of the “Whatever It Takes” model, which emphasizes self - determination, harm reduction, trauma-informed care, and psycho-social rehabilitation.
- Use effective communication skills such as active and reflective listening to build rapport and positively engage members.

**Data Collection**:

- Obtain consents and authorizations for participants in the Beverly Housing Supportive Services Program, as needed.
- Collaborate with each member to develop creative, measurable service plans aimed at improving overall well-being and housing stability; review and update quarterly and upon completion of goals.
- Develop and maintain complete, accurate, and current files with all required documents and data in agency records and electronic databases. Includes updates to changes in income and any demographic domain. Maintain confidentiality of participant files per HIPAA and all applicable guidelines
- Input any and all encounters with members into Apricot and Welligent EHR system to document member needs and treatment plan in a timely manner.

**SKILLS**: Ability to engage unhoused individuals in a meaningful and nonthreatening manner, evaluate and assess their needs and begin to develop trust enough that they accept support and services offered. Knowledge of or experience with DSM V diagnoses and of level of functioning, EHR documentation, EBP' s such as Motivational interviewing, Trauma Informed Care, Housing First, Harm Reduction, etc. Should have the ability to work independently and collaboratively with team members, take initiative, and embrace a solution focused practical approach. Must have knowledge of local resources and maintain a positive relationship with a multitude of community members in an effort to support the holistic care of each member.

**QUALIFICATIONS**:

- BA in psychology or social work, or 2 years of related work experience, with a
- minimum of 2 years' experience working with persons experiencing mental health disorders and homelessness, in a community setting.
- Case management experience is essential.
