**Referral Coordinator**

**RESPONSIBLE TO**:Supervisor Of Referral Coordinator

**GENERAL DESCRIPTION: P**rimary responsibility is to facilitate the process of referring patients or clients to other healthcare providers or services, ensuring continuity of care and effective communication between all parties involved. referral coordinator manages the entire process of referring patients to specialists, diagnostic facilities, or other healthcare providers as required by their primary care physician or healthcare team. Overall, the referral coordinator plays a pivotal role in facilitating the referral process, ensuring efficient communication between healthcare providers and patients, and providing high-quality customer service to enhance the patient experience.

**DUTIES AND RESPONSIBILITES**:

- Monitor and track referrals to specialists or diagnostic facilities.
- Provide regular updates to healthcare providers on the status of referrals.
- Process specialty referrals for health plan members using paperless protocols.
- Adhere to standard procedures and expectations outlined in clinical protocols
- Send letters to patients informing them of their referrals and explaining the reasons for them.
- Request reports from specialists regarding patient referrals, ensuring timely feedback to the referring provider.
- Send recall letters to remind patients of upcoming mammograms and Pap tests according to clinic protocols
- Utilize HIPAA protocols for processing referrals, ensuring confidentiality and security of patient information.
- Store and handle electronic referral packets securely, following HIPAA guidelines
- Maintain excellent customer service skills, both over the phone and in-person, for interactions with in-house staff and patients.
- Address patient inquiries and concerns regarding referrals in a professional and empathetic manner.
- Assist in screening patients for eligibility for special programs, following care coordination protocols Maintain accurate records of referral activities and patient interactions for reporting purposes

**QUALIFICATIONS**:

- Bilingual in Spanish and English.
- Knowledge in EClinicalWorks EMR
- Computer skills in Microsoft office products including Word; Excel, Outlook.
- Basic Computer literacy for ability to e-fax, print and scan preferred.
- Respond to patient needs promptly and effectively, exceeding expectations whenever possible
- Excellent communication and customer service skills.
- Strong organizational skills.
- Able to be flexible with roles.
- Understanding of medical terminology.
