**Overview**

You will be working on an interdisciplinary Assertive Community Treatment (ACT) clinical team working with individuals diagnosed with a Serious Mental Illness or General Mental Health diagnosis under Court Ordered Treatment. We are seeking a passionate individual to complete the team in our outpatient Healthcare Center in Phoenix, AZ. This individual will need to be professional, friendly, organized, compassionate and have a good understanding of housing resources within Maricopa county.

**Responsibilities**

Seeking an ACT Housing Specialist to join our interdisciplinary ACT clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our 51st Ave Healthcare Clinic.

$2,000 Sign-on Bonus

Full-Time Employed: 40 hours/week

**Location**:

- 51st Ave and Camelback Rd in Phoenix, AZ
- 23rd Ave and Dunlap Dr in Phoenix, AZ

Full Benefits Package, including 401K

Must have very flexible schedule as this team provides treatment 7 days/week, including weekends and holidays

Generous PTO/Sick Time (4+ weeks in year 1)

Starting at $18.38/hr

$2/hour ACT Differential IN ADDITION TO BASE WAGE, plus on-call stipend

**Duties Included**:

- Provides direct treatment, rehabilitative and supportive services that support independent living in a community setting to members.
- Works in conjunction with the patient and ACT team staff to assess patient needs and wants, assist the patient in locating housing of their choice, assists in accessing affordable, safe, decent, and permanent housing.
- Supports patients in protecting their tenancy rights regardless of participation in ACT team services.
- Participates in the development of individual service plans, acts as a patient advocate and liaisons with families and community agencies.
- Provides on-call crisis intervention services, as well as weekend and holiday coverage. (schedule flexibility required)

**Qualifications**

**Educational requirements**:

- 3 years experience in the delivery of behavioral health, **or**:

- Bachelor's in Behavioral Health or related field with 1 year of behavioral health experience, **or**:

- Master's Degree in Behavioral Health field
- 3 years working as a housing specialist _preferred_ - **minimum of 1 year required**:

- **Must have a good understanding of housing resources within Maricopa county**:

- Must be 21 years of age or older, with a valid Arizona driver’s license, and a minimum of 4 years driving experience and meet requirements of Terros Health’s driving policy
- Microsoft Word, Excel, PowerPoint, Outlook, electronic medical record, NextGen preferred
- Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members.
- Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions
- Basic understanding of the DSM-V Physical demands of this position are representative of those that must be met by an employee to successfully perform the essential functions of this job. Pay: $18.50 - $22.00 per hour

Expected hours: 40 per week
