The Families in Transition (FIT) program Mental Health Treatment Team is supported by the Family Partner/ICC, who works under the Program Manager's supervision. The Family Partner/ICC is a crucial member of the team, serving children between the ages of 0-21. They bring in the parents/caregivers' perspective and the family's unique cultural background to ensure proper care and support of the child. Furthermore, the Family Partner/ICC plays an active role in engaging with families receiving support from other service providers besides the ones offered by Family Paths. The Family Partner/ICC works closely with these service providers, arranging frequent meetings to discuss the family's progress and requirements.

In a nonjudgmental and respectful manner, the Family Partner/ICC closely works with families to assist them in accessing the services they require. They promote independence and advocacy skills within the family.

ESSENTIAL DUTIES & RESPONSIBILITIES

FAMILY PARTNER
- Help families access services and provide linkages to needed support services.
- Helps families increase their advocacy skills to obtain services and support.
- You will facilitate Child and Family Team (CFT) meetings with the child, family, County Workers, and providers present. You will also connect providers with the families to ensure regular participation in CFTs meetings.
- Collaborates with treatment teams, including County Partners at the Department of Family and Children Services and other providers, keeping them informed of progress and challenges as necessary.
- Assists families in increasing their support network on behalf of the needs of their child(ren).
- Serves as a role model and mentor for parents/caregivers whose children are receiving treatment services.
- Participates in mental health treatment teams as an equal member to strongly represent the parent’s/ caregiver’s perspective.
- Assists in the ongoing development and evaluation of the child’s goals.
- Provides case management, such as linking children and families to culturally and linguistically appropriate community resources and/or coordinating contacts with other service providers.
- Participates in outreach and community events.
- Participates in related trainings and workshops.
- Travels throughout Alameda County to meet the needs of the child and their families in their communities.
- Maintains an appropriate caseload and meets program billing goals as determined by the Program Manager.
- Completes all charting requirements/progress notes in a timely manner.
- Documents billable services according to County guidelines.
- Attends mandatory team and Agency meetings and trainings.
- Provide emergency and crisis response.
- Transportation of clients / family members to appointments and other community settings using transportation services such as Uber or Lyft.
- Performs other duties and assignments as required.

REQUIREMENTS, QUALIFICATIONS, SKILLS & ABILITIES:

- High school diploma or GED
- Primary caregiver of a child who has previously or is presently utilizing services (i.e: Special Education, Mental Health, Social Services, Regional Center)
- Basic computer skills/proficiencies: Microsoft Office Suite, the Internet, and workstation networks
- Good organizational skills and ability to handle multiple priorities.
- Ability to exercise good judgment and work independently on a day-to-day basis, legally and ethically, and to consult with a supervisor when appropriate.
- Ability to address differences and conflicts in a proactive manner.
- Knowledge of community resources in Alameda County
- Knowledge of trauma’s impact on families and ability to work from a trauma-informed perspective.
- Ability to support clients and to implement strategies that incorporate the family’s cultural values and strengths.
- Willingness to work with families with complex needs whose children are exhibiting mental health/behavioral issues.

Desired Qualifications:

- Bilingual, Spanish preferred.
- Experience with meeting facilitation
- Experience with in-home or community-based services for children and families
- Experience in social services and non-profit organizations
- Experience with EPSDT Medi-Cal funded programs is a plus

Special Requirements:

- Must pass criminal records clearance.
- Requires some driving on agency business, so must have access to reliable transportation, maintain a valid CA driver's license, current auto insurance, and a clean DMV record.
