JOB DESCRIPTION:
**Duties & Responsibilities include but are not limited to**:

- Verify all ADT events
- Track admissions/discharges and organize/circulate paperwork according to established procedures including, but not limited to:

- Receiving/forwarding hospital screens
- Medicare determination forms & ICD9 coding
- Gather information to fax to insurance providers as indicated
- Set up packets & folders for HMO admissions
- Process Medicare exhaust and discharge letters
- Bi-weekly update reports to insurance providers as indicated
- Requests specific medical information on a Resident as needed from other facilities, MD's, hospitals, etc. according to established procedures
- Maintains MDS Calendar
- Layer MDA/ADL CareTouch care path prior to 90 day review. D/C CareTouch following MDS completion.
- Maintains Resident rights, and Corporate Compliance standards
- Maintains HIPAA privacy, security, and confidentiality standards
- Follows all established policies and procedures
- Represents the organization professionally
- Interacts positively with Residents, family members, personnel, and visitors under a variety of conditions and circumstances

**Qualifications**:

- High School Diploma or equivalent
- Must have attention to detail and stellar organizational skills
- Able to communicate effectively while working in a fast-paced environment

**Requirements**:

- Minimum 18 years of age
- No findings or convictions of resident abuse, mistreatment and misappropriation of property
- Strong written and verbal communication skills
- Expertise and experience required for the task
- Demonstrated ability in management, leadership and interpersonal skills
