All services are free or low cost. We’re also one of the few Federally Qualified Health Centers in the nation with providers who specialize in primary care for LGBTQ people and with a research team working to advance the care and treatment of people in our community.

JOB SUMMARY: Under the supervision of the Manager of Integrated Care, the Psychosocial Case Manager III serves as an integral member of the health and mental health services team by providing psychosocial case management and overall support services to patients while ensuring that psychosocial case management services operate within industry standards and meets all contractual requirements and guidelines.

ESSENTIAL FUNCTIONS:

- Serve as part of a multidisciplinary care team to provide case management and patient care services.
- Work in collaboration with Medical Providers and other Health Services staff to maintain an effective and efficient program.
- Facilitate patient access to healthcare.
- Provide appropriate information on benefits, social services, and referrals while serving as an advocate.
- Network with other community agencies and organizations to establish referral linkages and develop resources for case management clients.
- Develop comprehensive benefits/social services plans for clients, providing support & guidance to clients in the implementation of services plans.
- Prepare and coordinate paperwork for public and private disability programs.
- Interface with Department of Rehabilitation and other related community agencies on behalf of case management clients.
- Maintain an active case load with clients requiring on-going case management services and maintain complete and accurate client files and program statistics as required by Agency, contract, and other related agencies.
- Act as a liaison between the clinic and Department of Mental Health Housing and Homeless Division.
- Provide referrals for housing resources based on clients’ psychiatric condition, social support network, substance abuse issues, and readiness for independent living.
- Gather/maintain available information related to housing placement activity including client placement, successes, and challenges.
- Make appropriate referrals to ancillary team members, i.e., Housing Case Management Staff, Nutritionist, Pharmacist, etc.
- Present clients in weekly case conferences.
- Consult with staff on service coordination and service plans for clients referred to ICP program.
- Maintain complete and accurate files documenting all care and services provided in the client’s health record.
- Interact in a professional manner with health services staff and patients.
- Exercise appropriate judgment and decision-making skills.
- Use standard office machinery including personal computers and printers in performance of duties.
- Assist in data auditing tasks, prepare reports as required, and submit in a timely manner.
- Participate in staff meetings, agency meetings, planning meetings, and other meetings as needed.
- Comply with requirements of OSHA, HIPAA, and other applicable regulations.
- Other duties as assigned.

JOB QUALIFICATIONS AND EXPERIENCE:

- Knowledge of or experience working with the LGBTQ community and familiarity with issues of particular relevance to LGBTQ people.
- A passion for the Center’s work and its mission to make the world a better place for LGBTQ people.
- Minimum 3 years of experience providing direct client case management services, preferably within a clinic setting.
- B.A. degree in social work, psychology, sociology, or a related field is preferred.
- Current AHA CPR certification.
- Experience in social work, psychology, sociology, or equivalent education.
- Knowledge of community resources and benefits programs.
- Understanding of the effects of chronic illness, mental illness, and substance use on the person, their family members, and significant others.
- Excellent verbal and written communication skills, including strong organizational, detail, and interpersonal skills.
- Excellent Computer skills and knowledge of Microsoft Office, word processing, Electronic Health Records, and other software systems.
