nts. - Assists in all youth related outreach and engagement requirements. - Attends all necessary and relevant administrative meetings/trainings and adheres to all program policy and procedural requirements. - Coordinates with all interagency departments and community agencies, to ensure care members proper demographic information, referrals and other HHCM documents are uploaded into FCM/MAPP. - Ensures all updated information from program related portals are transferred to EHRs as needed. - Retrieve information from clients, collaterals, and files in accordance with HIPPA as needed. - Assist HHCM leadership with review of all necessary documentation such as required consents, eligibility documentation, incident reporting procedures, and services tracking for billable services. - Assists in the carry through and follow up of compliant line adherence. *Skills, Knowledge, and Abilities:

- *-*Computer proficiency this includes but not limited to: Excel, and EHR software. *-*Ability to manage sensitive and confidential situations which require poise, tact, and diplomacy. *-*Excellent verbal, interpersonal and communication skills *-*Ability to work in a fast-paced environment with demonstrated ability to juggle and prioritize multiple, competing tasks and demands and to seek supervisory assistance as appropriate. *-*Ability to perform the essential functions of the job with or without a reasonable accommodation. *Qualifications/Requirements:

- HS Diploma required. Medical Assistant certification required. Associate degree preferred. - Due to the nature of the work environment work hours will be based on needs of program and may vary. - Our Agency operates in 2 locations, Staten Island and Brooklyn. The ability to travel between sites and perform assigned tasks and responsibilities may be necessary. - Performs other duties/responsibilities as assigned within scope of position. *Additional Information:
