**Description**

**Title**: Referral Coordinator - NHMP Neurology department - Poughkeepsie, NY. Full time/40 hours, 8 hour shifts, Monday - Friday, 8:30am - 5:00pm

**Summary**: Utilizing a high degree of accuracy in obtaining and verifying that all registration data; demographic and insurance information, is correct and up to date. Maintain organized, timely support for the multispecialty Medical Practice Care Team with communication of referrals between and with patients and insurance companies. Act as a liaison between NHMP and other community healthcare providers exhibiting and utilizing a high level of customer service skills in all aspects of the job. Responsible for performing a wide variety of clerical duties for administrators and practice staff.

**Responsibilities**:
1. Ensure complete and accurate registration, including patient demographic and current insurance information.

2. Review details and expectations about the referral with patients.

3. Answers patient calls and schedule patient appointments for multiple sites.

4. Facilitates “soft” transfers of patients to the appropriate office for any additional patient needs outside the scope of the office.

5. Maintaining contact with all NHMP Care Teams, regarding urgency, orders, and scheduling needs of the patients.

6. Be the system navigator and point of contact for patients and families, with patients and families having direct access for asking questions and raising concerns.

7. Act as a liaison between NHMP and other health care providers and facilities.

8. Maintain filing, charting, faxing, mailing and all other clerical functions need for this office.

9. Ensure that referrals are addressed in a timely manner.

19. Maintain ongoing tracking and appropriate documentation on referrals to promote team awareness and ensure patient safety.

20. Addresses patient complaints in a timely and effective manner; utilizing input from the Business Office Manager, providers and other staff as deemed necessary.

21. Maintains a working knowledge of all aspects of the office; including billing, bills, supplies, charting, workers compensations / no fault, corporate compliance issues and safety issues. Assists in providing directions to offices for patients.

22. Maintains confidentiality of all employees and patients; reports to upper management on any violations.

23. Other duties as assigned.

**Education Required**: High School Diploma/GED

**Experience Required**:

- 2 years physician office experience, Referral Coordination preferred
- Knowledge of ICD-9, CPT& HCPCS coding and medical terminology
- Strong interpersonal, telephone and computer skills
- Bilingual preferred

Salary Range:
$17.0667 - $32.4693 Hourly
