As a case manager, you will play a key role in assisting individuals with intellectual disabilities or autism, by accessing resources to assist them to live a full and inclusive life. You will do this by providing thorough assessments of strengths and needs, comprehensive planning of service needs, advocacy, outreach and resource coordination unique to every person served. **Requirements**:
- Bachelor's degree (required)
- Experience of 1 year working as an Adult Case Manager (required)
- Personable, self-directed, great written and oral communication skills, team player
- Comfortable advocating for individuals with various levels of disability in a variety of settings
- Travel required (must have valid driver’s license)
