**Summary of Duties and Responsibilities**:As a member of the patient care support team, the Medical Records Specialist is responsible for development, implementation and maintenance of patient medical records.

**Job Details and Essential Functions**:

- Assembles and maintains medical records in both paper and electronic systems, ensuring that all records are complete, and properly updated, with all required forms and documents signed and dated, according to regulatory standard/requirement
- Processes signed orders and certifications from the electronic medical record portal
- Participates in new hire orientation
- Reviews and audits medical records for accuracy and completeness and ensures security and confidentiality of medical records
- Cross trains to the Palliative Navigator role and supports the palliative program with clerical tasks and scheduling of visits
- Processes requests for patient information and processes requests for health information received from patient accounts
- Pulls, purges and archives medical records on an as needed basis
- Prepares weekly IDT agenda, creates re-certification forms for the Medical Director and notifies staff of pending re-certifications
- Participates in Quality Assurance Performance Improvement
- Manages outgoing/incoming faxes and assist with phone coverage as necessary to support clinical assistant and staff
- Participate on appropriate committees designated by the Director of Patient Care Services

**Knowledge and Skills**

**Required**
- Knowledge of medical terminology, medical office practices, release of records law
- Ability to maintain confidentiality and follow HIPAA and patient privacy laws
- Strong, clerical skills including typing, copying, scanning and faxing
- Ability to file in a neat and well organized manner
- Ability to communicate effectively, both orally and in writing
- Ability to consistently follow routine verbal and written instruction yet be self-directed
- Ability to use independent judgment to make sound decisions

**Desired**
- Public speaking, developing relationships with identified partners, groups, businesses with whom Hospice of the Foothills may have or need to establish affiliation

**Education & Experience**

**Required**
- High School Graduate or GED equivalent
- One year direct experience in medical records management
- One year direct experience in Medical Coding
- Excellent computer skills and understanding of computer environment and terminology
- Intermediate experience with MS Office (Outlook, Word, Excel)

**Desired**
- Equivalent to an Associate's Degree in Medical Information Technology (with course work in medical terminology, anatomy, physiology, disease processes)
- Registered Health Information Technician in good CE standing
- Two or more years direct experience in medical records management or medical clinic
- Previous experience in medical records management in a Home Health or Hospice
- Experience with NetSmart Software or eClinical Works

**License / Certifications**

**Required**
- Possess and maintain valid California driver’s license, a reliable automobile and evidence of appropriate liability insurance

**Desired**
- Certification as an Accredited Record Technician
- Registered Health Information Technician (RHIT) credential
- Medical Assistant certification

**Job Type**: Part-time

Pay: $20.00 - $23.00 per hour

Expected hours: 32 per week
