The Hampton - Newport News Community Services Board is seeking an Adult Service Coordinator to join our team in the Assertive Community Treatment (ACT) program. As an Adult Service Coordinator, you will play a critical role in ensuring the continuity of care for our clients, who may have co-occurring intellectual developmental disabilities and substance abuse issues. You will provide essential support to our clients by addressing their physical, financial, social, and personal needs. This role includes various responsibilities such as outreach, assessments, treatment planning, counseling, and crisis intervention, with the aim of improving the well-being of our clients and helping them lead more fulfilling lives.

As an Adult Service Coordinator, the principal duties and responsibilities include meeting with clients, families, and other care providers to assess client needs and eligibility for services. This involves preparing mental status and social assessments as well as other client information summaries. The role involves developing treatment and service plans for clients, making and documenting periodic reviews, and participating in client review meetings to provide input on other service coordinators' cases. Responding to requests for information about available services for mentally ill adult client populations is also essential. Advocacy is a key component, requiring the coordinator to represent the needs of clients both within and outside the system, acting as a liaison between the client, caregivers, and service providers.

Assisting clients in obtaining basic resources such as shelter, food, medical services, and disability benefits is another critical responsibility, along with helping with client relocations. The coordinator monitors client behaviors and progress, provides assistance towards attaining predetermined goals, schedules appointments, visits residences, and supervises medication compliance. Supportive counseling for clients and their families, emergency crisis counseling, and making referrals to appropriate service providers to coordinate service delivery are integral parts of the job.

The Adult Service Coordinator also participates in discharge evaluations and planning, prepares memos, correspondence, records, and reports related to service coordination activities, and consults with other agency professionals on difficult behavioral cases. Attending and testifying at court hearings and other legal proceedings involving managed clients is required. Additionally, the role involves developing and maintaining a housing resource list, conducting HIV risk assessments, and coordinating services for clients who are HIV positive and have mental health, intellectual developmental disabilities, and substance use disorder treatment needs. Finally, the coordinator conducts on-site urine drug screening for clients with substance use disorders.

**Service Coordinator Key Responsibilities**:

- Conduct outreach to identify and engage clients in need of services.
- Perform psychological and needs assessments to develop a comprehensive understanding of clients' individual challenges and strengths.
- Collaborate with clients to create and update personalized treatment plans.
- Facilitate connections with service providers and coordinate access to needed resources.
- Monitor clients' progress and adjust treatment plans as necessary.
- Advocate for clients to ensure their needs are met within the community.
- Provide counseling and crisis intervention when required.
- Offer daily living support to assist clients in achieving their goals.
- Supervise and monitor medication administration as needed.

**Service Coordinator Requirements**:
Bachelor's degree in Human Services, such as Social Work, Psychology, or Counseling, with one (1) year of experience in case management or service coordination services. Master's degree preferred. Must be registered with the Board of Counseling to practice as a Qualified mental Health Professional-A (QMHP-A) or register with the Board of Counseling to practice as a Qualified Mental Health Professional-T (QMHP-A trainee) within the first 30 days of employment. Valid Driver's Licenses
