The Independent Living Resource Center (ILRC) is a disability-focused non-profit providing services and advocacy to those experiencing all disabilities, of all ages, and all income levels. Our mission is to **promote independent living and full access for individuals with disabilities through advocacy, education and action in our communities. **The organization has been based in Santa Barbara since it opened in 1976 and serves the Central Coast Counties of; Ventura, Santa Barbara, and San Luis Obispo.

**About the Position**:
The CalAIM Housing Specialist provides housing navigation, deposit, and sustaining services to to Medi-Cal Members eligible for Community Supports Services. These services are provided in partnership with our local Manager Care Plans, CenCal Health and Gold Coast Health Plan. This positions supports Ventura and Santa Barbara County and its home office is based in Ventura.

**Essential Functions of the Job**:

- Follow protocols and practices in the provision of related services as required by ILRC;
- Performs intake, needs assessment, screenings, and enrollment of consumers for Community Support (CS) Services;
- Provide Housing Transition and Navigation, Housing Deposit, and Housing Tenancy and Sustaining Services as defined by CalAIM x ILRC;
- Maintain monthly communication with all consumers receiving CalAIM CS services;
- Provide person-centered counseling to identify individual goals and needs to inform consumers of service options;
- In conjunction with consumers, develop and implement individualized goals (ILPs/IHSPs);
- Utilize no wrong door policy when providing referrals, both internal and external;
- Advocate with consumers when access rights are limited or denied;
- Maintains detailed knowledge of programs, resources, and organizations in our catchment area that can support increased or maintained independence, including those of partner organizations within the CalAIM realm;
- Builds relationships with and maintains detailed knowledge of subsidized housing programs, resources, and organizations;
- Train consumers in housing search skills and access to housing benefits/programs;
- Maintain digital consumer files and document all services provided in a timely manner;
- Maintains documentation standards as required by the Program and PATH policies, including completing case file notes in a timely manner;
- Represent ILRC at community events and committees;
- Provide Spanish language access to consumers by translating materials and appointments with other staff;
- Attend and participate in staff meetings and any recommended trainings;
- Promote harmonious working relationships, participate fully as a member of working team, conduct ILRC business in a professional manner;
**Essential Qualifications**:

- Familiarity with human services programs affecting persons w/disabilities of all ages;
- Motivating interest in disability/independent living field;
- Excellent written and verbal communication skills;
- Ability to implement training various service delivery models;
- Ability to meet deadlines;
- Experience with service documentation;
- Computer skills including data, social media, and research;
- Reliable personal vehicle for work travel;
- Must maintain valid driver’s license and a satisfactory driving record;
- Ethical work practices: honest, dependable, team-oriented;
**Preferred Qualifications**:

- Personal experience with disability
- Bilingual English/Spanish
- Experience with CalAIM

Pay: $23.00 per hour

Expected hours: 40 per week
