**Sierra Winds is currently seeking a passionate Director of Supportive Services for our Independent Living residents that require additional healthcare-related services.**
The Supportive Services Director is responsible for evaluating the Independent Living needs of residents, assisting with the development of programs to meet those needs, teaching self-help techniques to ensure that the medical, social, and emotional needs and the general welfare of residents is met, and supervising department staff. Participates in the coordination process regarding resident admissions to the Health Center.
**ESSENTIAL JOB DUTIES**
- Evaluates the physical, emotional and/or social needs of residents.
- Responsible for promoting independence through directing, educating, and orienting residents in the use of appropriate adaptive equipment or techniques.
- Review living environment and recommends adaption to eliminate barriers.
- Assists in the orientation of all new residents to adjust to community living.
- Conducts resident clinics, educational and support groups as indicated.
- Maintains appropriate progress notes. Assures confidentiality of resident information.
- Maintains communication with physicians, families and appropriate staff regarding resident status.
- Acts as liaison for residents between facility, therapy agency, and home health agency.
- Assists residents and families by offering support in dealing emotionally with the aging process and illness.
- Assists and stimulates residents to participate in social and creative activities.
- Assists in coordinating services for residents being admitted to and discharged from Health Center.
- Monitors infections and accidents of residents. Notifies families and physicians of serious accidents or incidents.
- Supervises Supportive Services personnel.
- Monitors private duty personnel in accordance with community policies and procedures.
- Evaluate, compile, and maintain documents for contracted caregivers.
**ADDITIONAL DUTIES**
- Coordinates Supportive Services activities with other departments.
- Assists in interpreting and keeping current Supportive Services policies and procedures.
- Serves as a member of Resident Evaluation Committee. Attends routine meetings, department head meetings, and other meetings as assigned.
- Attends in-service training and education sessions, as assigned.
- Performs any other duties or assume any other responsibilities as directed by supervisor, or as is necessary to perform this job.
- On-call status twenty four hours a day, seven days a week. Must be willing to rotate on-call coverage, evenings, weekends and holidays.
**EDUCATION/QUALIFICATIONS/EXPERIENCE**
- An LPN licensed by the State of Arizona.
- Five years working in geriatrics in long term care, home health or other health care setting.
- Five years organizational or management experience supervising others preferred.
- Must have good English language skills and the ability to communicate well both in writing and orally.
- Must possess good organizational skills; able to work without supervision and constant direction.
- Must possess the ability to make independent decisions when circumstances warrant such action.
Pay: $70,000.00 - $75,000.00 per year
