**Position Summary**

RN Field Assessor is responsible for face to face assessing with members to evaluate the medical needs of the member to facilitate the member’s overall wellness and help them obtain the services they need to thrive. Addressing requests for services such as adult medical daycare, pediatric medical daycare, personal care assistant, nursing facility custodial requests, personal preference program and MLTSS program enrollment.
- Travel for this position is 75-100% within Camden, Cape May, Gloucester, Cumberland, and/or Atlantic Counties.
- Schedule is Monday - Friday, 8:00am - 5:00pm.
- 10-15 assessments minimum per week dependent on need, may be more. Assessments take into account information from various sources to address all conditions including co-morbid and multiple diagnoses that impact functionality. Assessments include the member’s level of work capacity and related restrictions/limitations.
- Using a holistic approach assess the need for a referral to clinical resources for assistance in determining functionality.
- Consults with supervisor and others on team in overcoming barriers in meeting goals and objectives. Utilizes case management processes in compliance with regulatory and company policies and procedures.
- Utilizes interviewing skills to ensure maximum member engagement and discern their health status and health needs based on key questions and conversation.
- The RN Assessor will also be expected to mentor new hires on the team, once, a level of proficiency has been attained in their role.

**Required Qualifications**
- Must reside in Camden County, NJ or neighboring county.
- 2+ years clinical practice experience e.g., hospital setting, alternative care setting such as home health or ambulatory care required
- A Registered Nurse that must hold an unrestricted license in their state of New Jersey & residence, with multi-state/compact privileges and have the ability to be licensed in all non-compact states.
- Successful completion of the NJ Choice certification is a contingency of employment. (As per NJ Division of Aging Services, Office of Community Choice Options guidance, an exam score of 80% or higher is required, along with successful completion of the State training modules, and field mentoring component.”)

**Preferred Qualifications**:

- Assessment and Case management and/or discharge planning experience preferred
- Managed Care experience preferred
- Crisis intervention experience preferred
- NJ Choice certification preferred
- Clinical interviewing experience
- Comfortable in socioeconomic setting

**Education**
- Associates of Nursing and above

**Pay Range**

The typical pay range for this role is:
$63,200.80 - $136,600.00

This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
