**Job Requirements**:

- Applicants must be 18 years of age, possess a valid driver's license, and have access to a vehicle.
- Ability to read and comprehend written reports/regulations and provide written documentation in a clear and understandable manner.
- Ability to perform basic mathematical calculations.
- Ability to communicate effectively in daily Progress Notes of consumers
- Ability to use EVV software such as Therap for entering and maintaining progress notes.
- Must successfully pass all pre-employment screenings including criminal background, FBI, employment verification,
- Must successfully pass and physical examination and Mantoux (TB) or QuantiFERON-TB Goldunder ODP 6400.151 requirement
- Must maintain current physical examination and Mantoux (TB) or QuantiFERON-TB Goldevery 2 years after original physical and Manxtoux under ODP 6400.151 requirement without lapse to maintain employment with Agape Human Services

**Job Duties Include**:Under direction of a Program Specialist/House Supervisor or other supervisor/manager in Agape Human Services does related work as required:

- Assist person supported with daily living requirements including but not limited to meal preparation, medication administration, and personal care.
- Manage significant social and behavioral challenges in a supportive, non-restrictive manner. Experience supporting with individuals with intellectual disabilities, mental health challenges, and/or autism is highly desirable.
- Administer Medication to Consumer by ODP Med Certified Staff.
- Provide transportation and scheduling assistance for medical appointments, local community outings or scheduled appointments, therapy sessions,and social outings in direct support staffs personal vehicle. ·Document and account for all financial transactions related to person supported.

Implement and document all supports as specified in the Individual Support Plan.
- Ensure Consumers’ health, safety, dignity and individual rights at all times while enhancing the quality of the individuals’ everyday life.
- Participate in the development and implementation of each Individual Support Plan (ISP).
- Provide services to each person in their home and/or their community according to their Individual Support Plan (ISP).
- Participate in implementing the Individual Support Plan (ISP) designed to enhance the individual’s social, emotional, cognitive, physical, and independent living skills.
- Assist individuals in the development of appropriate behavioral, social, money management, housekeeping, hygiene, and other skills based on outcomes in the Individual Support Plan (ISP).
- Adhere to Department of Public Welfare (DPW)/Office of Developmental Programs (ODP) Regulatory requirements.
- Complete and submit all daily progress notes and time sheets in a timely and professional manner.
- Collaborate and cooperate with involved Federal, State, County agencies, family members, natural supports network and other service providers; participate in meetings and provide relevant information.
- Work independently with mínimal supervision in the group home, person’s home or day program and/or local community.
- Ensure that the services are provided incorporating the Principles of Everyday Lives and Positive.
- Approaches in the least restrictive setting and with the utmost attention to their health, safety, rights and dignity.
- Provide skilled instruction for the person in an appropriate setting according to each person’s Individual Support Plan (ISP).
- Utilize local community resources in order to promote community integration, independence, and interdependence.
- Provide support in medical related emergency situations according the CFP+ Consumer Medical Emergencies Policy.
- Review and assess, initially and on an ongoing basis, the Individual Support Plan (ISP) in order to provide appropriate services to each person.
- Provide support and assistance to individuals in arranging for medical care, when responsible to do so, as per the Individual Support Plan (ISP) while following health care recommendations.
