**Role description**:
This position is responsible for supporting Care Managers. The Care Coordinator Support Specialist works collaboratively with interdisciplinary care teams that may include the member's PCP, specialists, Social Worker, Behavioral Health Specialist, and community resource contacts, while functioning with professional autonomy under the supervision of the Care Coordination leadership team. Individuals in this position demonstrate cultural sensitivity, effective communication, and motivational interviewing skills.

**Responsibilities**:

- Support the caseload of Care Managers
- Updating patient information and making changes in electronic medical record platform
- Be the system navigator and point of contact in collaboration with Case Managers for members and families
- Develop relationships with local agencies and community resource organizations
- Conduct calls introducing members to the Care Coordination program, setup initial (and annual) Care Coordinator interactions
- Scheduling appointments
- Assisting patients with referral scheduling, when needed
- Navigation of multiple platforms for routing information and documentation for clinical staff, uploading documents into the platforms, as needed
- Manage phone lines by answering, taking messages, and conducting outbound calls, as instructed
- Provide administrative support
- Assist with inbound and outbound mail and faxing
- Assist new patients with paperwork
- Assist with community room events, as needed
- Conduct outbound calls as necessary
- Triage inbound calls with appropriate escalation or resolution
- Maintain HIPAA standards and confidentiality of protected health information
- Other duties, as assigned

**Qualifications**:

- High School Degree required
- Minimum of 6 months of experience in a clinical, mental health, or managed care setting is preferred
- Demonstrated customer-focused interpersonal skills to interact in an effective manner with practitioners, the interdisciplinary care team, community agencies, members, and families
- Ability to work autonomously, follow through with delegated tasks, and be directly accountable for deliverables
- Outstanding verbal and written communication skills
- Welcoming and nurturing attitude toward our patient population
- Experience in a customer service setting
- Ability to work independently and maintain flexibility in a fast-paced environment
- A high-level of accountability and responsibility for the outcome of care
- Excellent interpersonal and facilitation skills
