**Job Profile Summary**

Completes assigned referrals for assigned department(s). Makes appointments directly into CMIS for the appropriate FHCSD site or service. Schedules translation services as needed and makes reminder calls as assigned. Provides excellent patient-centered communication and education regarding referral processes.

**Job Roles**
- Accurately completes referrals to include ICD-9 and CPT codes, following established department protocols. Makes initial appointments directly into CMIS; gives appointment cancelation information.
- Actively tracks follow-up and referral pool(s); schedules patient services as required.
- Communicates to clinic staff status of pending referrals using CMIS updates and chart notes; indicates referral status changes as they occur.
- Enters required data, including basic insurance and screening information. Provides fee information for self-pay patients.
- Follows established protocols for patients who indicate they are in crisis.
- Makes assigned reminder calls.
- Schedules interpreter services as required.
- Submits accurate and timely data about referrals and appointments by site.
- Performs other duties as assigned.

**Education/Certifications/Licenses/Registrations***
- 1 year of experience in an ambulatory care setting required.
- High school diploma or GED equivalent required.
- Successful completion of a medical terminology course preferred.
- Or equivalent combination of education and experience that provides the skills, knowledge and ability to perform the essential job duties, and which meets any required state or federal certification requirements.

**Experience/Specialized** skills (including Language)**
- Ability to accurately complete data entry at relatively high speed, utilizing grammar and spell check software.
- Ability to explain services provided in assigned department(s).
- Ability to work effectively with patients from diverse social, cultural and economic groups.
- Able to maintain good working relationships with employees, coworkers, and departments.
- Basic computer literacy ability to comply with department needs and expectations.
- Bilingual in Spanish/English preferred; may be required in some positions.
- Good interpersonal and customer service skills.
- Good written and verbal communication skills.
