**Job Purpose**:This is a professional position that is responsible for the care and treatment of adults living with a mental illness. The Case Manager and client will develop and implement the treatment plan, which will include the necessary follow-up, case management, and ongoing interventions.

**Essential Job Functions**:

- Develops a treatment relationship with the consumer and formulates treatment plans on their caseload that will detail treatment to be provided.
- Provides psychosocial rehabilitation, symptom monitoring & management, and supportive counseling to consumers on caseload and participate in “team case management”.
- Monitors clients’ psychiatric medication regime and compliance as directed by the physician.
- Provides crisis intervention and supportive counseling to consumers who are in crisis.
- Maintains a treatment-oriented environment (under supervision of program supervisor) and assures consistency in carrying out treatment objectives.
- Coordinates utilization of various community resources, day programs, and outside health care providers.
- Assists clients in financial planning and the development of income resources.
- Coordinates consumer access to all entitlement programs and will act as protective payee for consumer’s Social Security benefits when appropriate.
- Provides treatment-related education and liaison with the client’s family and natural support.
- Assists client in securing adequate housing and prompting and training for skills necessary to keep housing.
- Coordinates the efforts of all professionals and paraprofessionals involved with each client.
- Formulates appropriate discharge plan with the assistance of client and program supervisor.
- Accurately documents and maintains case files as required by agency policy, government regulations, and applicable local, state, and federal codes.
- Meets department-designated monthly billing quotas, EHR note entry within 24 hours, biannual treatment plans and other funder required documentation.
- Attends and participates in staff meetings, in-service training seminars, and conferences as required.
- Adheres to agency and Milwaukee County(funder) policies and procedures while carrying out daily responsibilities.
- Collaborates and assists with all levels of personnel within the program to achieve common goals.
- A personal vehicle, a valid Wisconsin driver’s license and insurance in conformity with agency limits
- Performs special assignments and or projects as assigned.

**Required Education, Experience, Certifications, Licensure and Credentials**:_(Where appropriate, education and/or experience may be substituted)_

**Minimum Required Education**:Bachelor's Degree is required in behavioral science or related field.

**Minimum Required Experience**: Preferred clinical experience with persons with a chronic mental illness

**Required License - Certification - Registration**:

**Travel outside of the office**:90% travel in the communities we serve.

**Required Valid Driver’s License**: Valid Wisconsin driver’s license.

**Required Auto Insurance**:vehicle liability insurance in accordance with Agency policy.

**Knowledge - Skills - Abilities**:

- Access to a personal vehicle
- Knowledge of Milwaukee County TCM, CCS and/or CSP/ACT Case Management.
- Experience with individuals with dual diagnoses is preferred.
- Experience with motivational interviewing is preferred.
- Prefer 1,000 hours of post-graduate experience working with adults with severe and persistent mental illness.

**Visual Acuity**: The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal.
