**Responsibilities**:

- Conducts a psycho-social assessment for each individual/family assigned. The assessment should identify patient and family strengths, coping styles and problem areas within the context of their diagnostic, social, financial, cultural and psychological situation.
- Counsel patients providing culturally relevant treatment, crisis intervention and transition services.
- Assists families in integrating the medical, social and emotional dynamics of the disease/treatment into the context of their current life situation. Identify environmental impediments to patient progress through interviews and review of patient records.
- Documents social work interactions in the EMR in a timely manner.
- Coordination and linkage with community resources and agencies relative to health, protective services, crisis intervention, disability, schools, etc.
- Provides support for patients and their families, assisting with emotional, personal, psychological and socioeconomic factors, along with adjustment to illness issues. This includes the entire spectrum from diagnosis throughout the treatment phase, as well as the bereavement process, if necessary.
- Provides ongoing coordination of services to assist children with long-term chronic illnesses in managing the impact of the illness through the various phases of childhood, adolescence and/or transition into adulthood.
- Responds to the needs of culturally diverse and socially, intellectually or physically challenged patients.
- Investigate child abuse or neglect cases and take authorized protective action when necessary.
- Organizes, plans, and manages time effectively.
- Meets position requirements and performs essential functions.
- Completes all mandatory training as required.
- Performs other duties as assigned.

**Qualifications**:

- Licensed Clinical Social Worker (LCSW) credential required in the state of Montana
- Ability to speak, read and write effectively
- Ability to work independently without direct supervision
- Ability to coordinate, analyze, observe, make decisions, and meet deadlines, in a detail-oriented manner
