**About the position**:The LVN is responsible for observing the patient, reporting these observations, and documenting observations while providing direct patient care and treatment towards hospice patients in a hospice house setting in compliance to the plan of care to help meet positive patient care outcomes, that promotes quality, continuity and safety of a patient’s care as a member of the interdisciplinary team under the supervision of a registered nurse.

**Responsibilities**:
30% Provision of Care
- Provides direct patient care under current nursing practice following a comprehensive assessment and the plan of care
- Uses equipment and supplies effectively and efficiently
- Provides appropriate pain/symptom management under the direction of RNCM and HMD
- Observes patient’s response to treatments/medications and reports to RNCM and HMD
- Provides patient and family/caregiver education and information pertinent to plan of care
- Provides accurate and timely documentation of patient care services
- Updates medication list in EMR and reconcile facility MAR
- Carry out medication orders and refills
- Completes facility forms including MAR, centrally stored medication list, narcotic log sheets
- Completes facility checklist upon patient admission and death
- Organize patient binder, patient file upon admission and death
- Perform MAR audits
- Organize medical supplies in designated areas of the house
- Completes other duties as assigned

30% Collaboration & Communication
- Participates in the coordination of hospice services and reports identified needs to the Program Director and interdisciplinary team
- If unable to perform a certain task, report to the Case Manager immediately
- Takes and records temporal and axillary temperatures, pulse, respiration and blood pressure when ordered with appropriate completed/demonstrated skills competency
- Reports on patient’s condition and significant changes to house LVN and case manager
- Respectful of patient and family/caregiver environment and patient’s personal needs
- Compassionate attitude toward the care of the terminally ill
- Ability to develop and maintain rapport with patients and families
- Ability to organize and prioritize workload independently and to carry out directions with maturity
- Demonstrates ability to interact graciously and relate effectively with patients, families, and interdisciplinary team

30% Compliance & Risk Management
- Adheres to the organization’s documentation and care procedures and standards of personal and professional conduct
- Participates in quality assessment performance improvement teams and activities
- Completes charting in EMR in a timely manner per Mission Hospice policy
- Completes and sends shift reports to interdisciplinary team in timely manner
- Attends scheduled staff meetings and In-services
- Work weekend shifts as assigned
- Completes all requirement for employment in a timely manner
- Comes to work on time for scheduled shift
- Other duties as assigned

10% Professional Development
- Maintains required competencies and certification to perform job
- Assumes responsibility for professional growth and development, maintains and upgrades profession knowledge through attendance and participation in continuing education, in service classes and staff meetings
- Express understanding of self-care methods

**Qualifications**:

- Current CA LVN certification
- Graduate of an accredited LVN program
- Current CPR certification
- One year experience in hospice, home health, skilled nursing facility setting preferred
- Valid California Driver’s License and an automobile that is insured in accordance with state requirements
- Computer proficiency in Microsoft Office and willingness to learn new software as needed
- Team player with excellent organizational, communication, and problem-solving skills
- Compassionate attitude toward the care of the
