Job description

Premier Home Health Care Services, Inc. is a leading established Home Health Care Agency. We currently have an opportunity for an Intake Specialist in South Side Chicago.

The CCU Intake Specialist is responsible for the scheduling and tracking of all CCP, MCO, and HDM initial visits, Interims, CERAs and TSIs in Arrow and the Arrow Issue Tracker. This individual will maintain quarterly communications with participants and will update the participant’s record with all new contact information.

**Qualifications**:

- Minimum High School Diploma (or equivalent).
- Minimum of one-year experience in health or human services.
- Strong attention to detail and follow-up skills.
- Ability to use computerized systems.
- Ability to balance multiple tasks and manage conflicting priorities.
- Minimum of one-year in customer service skills.
- Possess general knowledge of office procedures.
- Bilingual English/Spanish required.

**Essential Job Responsibilities**:

- Schedule all Initials for participants and document all communication with participant in their Arrow Profile.
- Complete the Pre-300 Series Wellness Survey and upload the document to participant’s Arrow Profile.
- Schedule and track all Interims, CERAs, and TSIs for new and current participants, document all communication with participant in their Arrow Profile, and enter the issues into the Arrow Issue Tracker.
- Complete Care Management Surveys by following workflow protocol and upload proper documentation to participant’s Arrow Profile.
- Be the main point of contact for the enrollees assigned and participate in the care planning process and development of Care Plans based on assessments received from the Care Manager.
- Participates in data entry support for all State systems as needed. Required to meet all timeframe measures.
- Maintain required documentation.
