As a Family Support Specialist at Four Oaks, you will play a vital role in providing supportive services that enable children to safely remain with their family, be reunified with their family, and/or achieve alternative permanency. This position involves provision of family support, care coordination, family functioning interventions, supervision, and safety monitoring.
Key Responsibilities:
- Maintain regular contact with referring workers, families, and identified collaterals to achieve permanency goals.
- Coordinate interdisciplinary treatment, maintaining weekly contact with therapists and program coordinators on all assigned cases.
- Document service delivery, including objectives of treatment and/or intervention strategies.
- Understand and utilize defined assessment tools in working with all cases.
- Facilitate community resource planning, case progress evaluation, and permanency planning.
- Coordinate interdisciplinary crisis planning.
- Participate in discharge planning and documentation.
- Schedule and facilitate quarterly staffing on all assigned cases.
- Acknowledge and address the treatment needs of each case.
