* Work collaboratively to monitor and address clinical quality and gaps in care
* Provide clinical and educational assistance to support project initiatives
* Demonstrate communication skills necessary to interpret age-specific responses
* Participate in performance, improvement, and quality assurance activities and related meetings
* Provide clinical and educational support to meet program goals

*Required Licensure*
* Current NY State Board Certified Nurse Practitioner Licensure

*Required Experience*
* 2+ years practicing experience as a Nurse Practitioner
* Computer literacy and the ability to navigate new and emerging technology
* Highly developed written and oral communication skills; strong interpersonal skills
* Ability to work in a fast-paced and changing environment with proven ability to work with community groups
